Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced 3 - Year Required Annual inspection to the Family Child Care Home on 12/05/2024 at 10:00 AM. LPA met with Licensee, Jerrilyn Cordett, at 10:09 AM and observed seven (7) children in the backyard play area. LPA toured the backyard prior to entering into the home. LPA observed eight (8) children with Assistant (A1) at 10:32 AM. Upon file review, LPA observed seven (7) infants in care. Licensee is licensed for a large family child care home with a capacity of 14 children. Two (2) of the 14 children must be school age, in which one child is enrolled and attending Kindergarten and one child at least six (6) years old. When there are no school age children in care, the capacity of children in care reduces to 12 children, in which no more than 4 may be infants (under the age of two (2) years old) when an Assistant is present. During today's inspection, LPA observed 15 children in care in which 7 were Infants (under the age of two (2) years old; which causes an immediate risk to the health, safety, and personal rights of the children in care; therefore, facility is operating out of compliance (Type A deficiency cited refer to LIC 809-D). During inspection, LPA observed three (3) children, in which two (2) were infants, picked up by parent/guardian. All adults present, working or volunteering during today's inspection have a criminal record clearance or exemption.
PHYSICAL PLANT
All areas on the facility sketch were identified. This a one story home with three bedrooms, two bathrooms, kitchen, dining room, living room and attached garage. Off limit areas of the home are all three bedrooms and master bathroom located to North of the home. Bedrooms are made inaccessible by closed doors during operation hours. The primary childcare area is in the converted garage. LPA observed cubbies labeled with child's name, toy trucks/cars, kitchen play set, books, other age-appropriate toys and changing table station in converted garage. The living room is utilized for circle time and as a napping area. LPA observed, screened fireplace, napping cots and television in the living room. The kitchen is utilized as an alternate snack and meal time area on rainy days. The children utilize the bathroom at the entrance of the home. LPA observed cabinet under kitchen sink and drawers with safety locks and latches. Meals and snacks are provided by child's parent. LPA advised Licensee to ensure food containers and water bottles are labeled with child's name and properly stored or refrigerated. PAGE 1

Outdoor play is conducted in the backyard of the home. The outdoor play area is fully fenced in and free of hazards. LPA observed a children's basketball play set, children's size table and bench set, swing and slide apparatus and age-appropriate toys in the outdoor play area. LPA advised Licensee that children in care shall be supervised at all times.

Children eat snacks and meals outdoor as weather permits. LPA observed tented meal area with children's size table and bench set.

Hours of operation are Monday through Friday, 8:00 AM to 5:00 PM. Parents drop off child at the front door and pick-up at the side gate to the South of the home.



The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children in care. The home has working telephone service and telephone number on file with the Department was confirmed and verified. All poisons, to include, detergents, cleaning compounds, medication and other hazardous materials are inaccessible to children in care. There is a fully charged fire extinguisher that is at least a 2A:10BC in the home. There is at least one working carbon monoxide and smoke detector in the home. Centralized heat and air ventilation system is used as heating and cooling source. LPA observed safe and age-appropriate toys and play equipment in the home.

According to Licensee, there are no weapons, firearms or ammunition on the premises; none were observed by LPA. Licensee was informed if obtains, firearms and ammunition must be stored and locked separately.

Per Licensee, there is a small pet dog in the home that is inaccessible to children in care during operation hours. LPA heard pet, however, did not observe pet during inspection.

LPA observed current Pediatric CPR and First Aid card for Licensee with expiration date 08/17/2026 and for Assistant with expiration date of 11/11/2025. LPA did not observe current renewed and completed Mandated Reporter training for Licensee. LPA advised Licensee to complete Mandated Reporter training and place certificate of completion in file. LPA observed Mandated Reporter training certificate for Assistant with completion date of 10/30/2023.

LPA observed the following required postings posted in the primary childcare room:
  • Facility License
  • PUB 394 Notification of Parents' Rights
  • LIC 9148 Earthquake Preparedness Checklist

  • LPA advised Licensee to perform a walk through of facility and complete LIC 9148 and re-post once completed.

LPA reviewed the following documents during today's inspection:
  • LIC 610A Emergency Disaster Plan
  • Verification of Disaster and Fire Drills (last conducted 09/06/2024)
  • LIC 9040 Facility Roster (LPA advised Licensee to complete date of children no longer enrolled)


The facility was not operating in substantial compliance during today’s inspection on 12/05/2024. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, deficiency was cited for non-compliance of capacity license terms.

The following was thoroughly discussed with Licensee:

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. PAGE 3

During the exit interview, the LICENSEE, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

LPA, Keyona Scott, informed licensee, Jerrilyn Cordett, that this report dated, 12/05/2024, documents one (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA, Keyona Scott, informed the licensee to provide a copy of this licensing report dated 12/05/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with the licensee Jerrilyn Cordett.
