On 8/14/2024, Licensing Program Analyst (LPA) Jeanine Lipsey made an unannounced visit for the purpose of delivering findings regarding the above allegation. LPA met with, Director Rosa Nepomuceno.

Census: There were 61 children being supervised 10 staff.

Pertaining to the allegation the allegation “Facility staff do not ensure children are kept in clean dry clothing at all times”.

Per the Reporting Party (RP), at picked up, C1’s, poop was down his leg and up his back. This happened 3 or 4 times from Feb.12th through Feb. 26, 2024. Staff disclosed they gave mom a call asking the mom to get bigger pull-ups and for C1 to wear sweats to make it easier to pull up because the jeans were too tight

On 8/14/2024, Licensing Program Analyst (LPA) Jeanine Lipsey made an unannounced visit for the purpose of delivering findings regarding the above allegation. LPA met with, Director Rosa Nepomuceno.

Census: There were 61 children being supervised by 10 staff.

Pertaining to the allegation the allegation “Facility staff did not ensure treatment plan was followed for child in care”.

Per the Reporting Party (RP), the staff did not follow the treatment plan of, the Child must wear a hat and wear sunscreen when going outside for one year. Per RP they alledgly provided the treatment plan to the facility and filled out required LIC 9221 form; PARENT CONSENT FOR ADMINISTRATION OF MEDICATIONS AND MEDICATION CHART.

 RP did not observe sunscreen on the child’s face at pick up and asked the child if teachers applied sunscreen and the child said no. LPA observed the LIC 9221 & medication in the director’s office for children, but no documentation of medications being given to any children. Per Staff #3, the parents fill out the LIC 9221, that they keep in the office, and at the scheduled time they give the medicines and write the time down on the form. Per Staff #7 they follow the protocol for giving medications, but sometimes do not write the times down on the form. Per staff #1 there is no record of a form from the doctor nor a LIC 9221 regarding applying sunscreen at this time. Per RP when C1 was dis enrolled both items were returned unused, and the hat was still new with the original creases.

Based upon observations and evidence obtained during this investigation, the allegation “Facility staff did not ensure treatment plan was followed for child in care”, has been determined unsubstantiated. A finding that a complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred.

Notice of site visit was given and must remain posted for 30 days.

Staff disclosed the child would have poop on them 2 or 3 times in a week and believes by the jeans and pull ups being too small, may have caused the poop to be on the child. Per Staff, once the parent bought the new size of pull ups and sweats, the accidents stop happening.

Based upon observations and interviews obtained during this investigation, the allegation of “Facility staff do not ensure children are kept in clean dry clothing at all times”, has been determined to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standards has been met.

Notice of site visit was given and must remain posted for 30 days.
