On 5/28/25 at 8:40am Licensing Program Analysts (LPA) Jeanine Lipsey and Laticia Thompson conducted an unannounced case management licensee-initiated inspection for the purpose of a capacity increase. LPA’s met with Director Rahmath Khalid and announced the reason for the visit. The childcare center operates Monday – Friday 7AM-6:00PM. The licensee is requesting an increase to their infant license program from 8 to 13 infants, ages newborn to 24 months. The visit is to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 8 infants present being supervised by 3 staff during this inspection. The fire clearance was approved on 4/29/25.
Indoor measurements are:

The Indoor measurements contain a total of 442.06 which will not accommodate Directors request for 13 infants. Based on today’s measurement the facility capacity limit for indoor space is 12 infants.

Outdoor Play measurements are:

The outdoor activity space measurements were not measured due to the facility was not prepared for the capacity increase. A chain link fence was present separating the new space from the old space. The play yard space was not clearly defined.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The smoke/carbon monoxide detectors were tested and working properly. Staff restrooms were not separated from Preschoolers Per Director, the staff and the children share the same restrooms staff has access to a separate stall. The facility has filtered water dispensers where children can fill their personal water bottles for outdoor and indoor activities. LPA observed First aid kit with the required materials including a handheld digital thermometer. Medication is stored in a box located above a 5 foot cabinet out of reach of children. Ill children use the directors desk area for isolation. LPA observed trash cans with tight fitting lids. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in locked cabinets.

The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects or debris. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. There is a sufficient quantity and variety of age- appropriate play equipment in good condition. Tables and chairs were observed to meet the needs of the children. There are cubbies in all classrooms for storage of children clothing and personal belongings. There are 5 cribs for napping but not enough to accommodate the current capacity of infants. The classrooms have central air for heating and cooling.

The required postings are posted in an area were parents and visitors can review.

The facility provides lunch and snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request. There is a refrigerator located in the kitchen that has a thermostat to regulate the temperature.

The facility does/ not provide transportation. Per Licensee, there are no Firearms and other weapons on the premises.

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space has artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed to be maintained in a safe condition and is free of hazards. There was not adequate shade in the outdoor activity area.

Facility Representative/Director stated there are not any bodies of water on the facility grounds. LPA did not observe any bodies of water on the premises.



The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA's printed out regulations for the director to read and understand.

An exit interview conducted and report was reviewed with Director Rahmath Khalid. A notice of site visit was given and must remain posted for 30 days.
