25 | Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced annual required inspection to the above facility on 05/14/2024. LPA arrived at the facility at 08:56 AM, identified self and met with Hamlet Tadevosyan, Licensee’s Spouse and Staff #1, who guided analyst on a tour of the inside and outside of the facility. There were 8 children present upon arrival. Gohar Vardanyan, Licensee arrived shortly after. LPA provided Licensee with a copy of the LIC125 Entrance Checklist to help facilitate the inspection. Facility operation hours are Monday to Friday from 08:00 AM to 5:00 PM.
The family child care home is a one-story studio ADU dwelling, includes combined living/dining/bedroom/kitchen and 1 bathroom. LPA toured the home inside and outside and a census was taken. Current facility sketch reviewed, and Licensee confirmed that combined living/dining/bedroom/kitchen and bathroom (located near entrance) are used for providing care and are accessible to children. The indoor combined living/dining/bedroom area is used for child care activities, eating, and sleeping, the area is set up with children’s tables, children’s chairs, play items, toys, books, children’s cots and bedding. The outdoor area includes a fenced play area that is shared with the second home on the property. The children use the bathroom near the entrance and was observed to be safe and sanitary. The licensee provides food for children in care. The kitchen is off-limits and made inaccessible by use of child proof locks. There is telephone service via a cellphone that is used, and the cellphone stays at the facility during operation hours.
Per Licensee, there are no areas off limits to children and parents inside the home. All areas identified that are accessible for children to use were inspected for safety, comfort, and cleanliness.
The second home on the property is off limits to children and an outside area next to the second home is off limits and is made inaccessible using a child gate secured to the wall and building.
