On 5/24/2023 Licensing Program (LPA) Judy Laureano arrived at above mentioned facility for the purpose of delivering finding for the above mentioned allegation. Upon arrival, LPA met with Lisa Pertula and discussed the purpose of the visit. LPA toured the facility booth indoor and outdoor and a census was taken; 33 children and 9 staff members were providing care and supervision.

On 3/27/2023 Licensing Program Analyst (LPA) Laureano arrived at Blue Oak Creative School House on 1501 17th Street, Santa Monica, CA 90404 for the purpose of investigating the above-mentioned allegation. LPA toured the facility and observed 22 children in care and 6 staff providing care and supervision. Executive Director and office manager were also present during today's inspection.

On 3/27/2023 LPA interviewed staff and director. On 5/8/23 LPA interviewed P3, P4, P5. On 5/11/23 LPA interviewed P1 and P2. On 5/24/2023 LPA concluded interviews of all relevant parties. LPA reviewed the following: children's roster, discipline policy, staff roster, “ouchie reports/ incident reports”, parent handbook and other relevant documentation LPA reviewed third party specialist Psychotherapist and behaviorist observation reports that concluded no personal rights were being violated. No information was disclosed that children's personal rights were violated, therefore the allegation is UNSUBSTANTIATED, meaning although the allegations may have happened or are valid, the preponderance of the evidence standard has not been met.

Exit interview was conducted and a copy of the report was provided.
