13 | On 11/1/24, at 8AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced site visit to this facility to deliver findings on the above-mentioned allegations. Upon arrival, LPA met with Licensee / Virginia Levitt, who guided LPA on a tour of the facility. There were zero (0) children present at the time of arrival but shortly after, children began to arrive at the day care. A total of 11 children were observed with 2 staff. LPA explained the purpose of today’s visit.
During the course of the investigation, interviews were conducted, staff and children's rosters were obtained/reviewed and pictures taken of the physical plant.
Per Reporting Party, Facility furniture is in disrepair and Facility is unsanitary.
During the interview with the Licensee, it was disclosed that during the month of August, (date unknown) a child was sitting on a rusted chair when the chair collapsed causing the child to fall. Per the Licensee, the |
