32 | During an interview with the Licensee and Co-Licensee, through their own admission, they forgot to include the facility License number when they were advertising their facility to the public. Licensee and Co-Licensee stated that they have a website, car signage, gate signage, and Facebook profile that they use to advertise their facility to the public. The Licensee and Co-Licensee disclosed that the facility advertising on the facility website bfkidsacademy.com was updated to include the facility license # 197494835 on 09/18/2025. LPA observed advertising signage on a vehicle and on the facility gate (photo taken). Licensee and co-Licensee updated two (2) advertising signage on the door of the vehicle and the back window to include the facility number (photo taken). Also, Licensee and Co-Licensee updated two (2) large banners on the facility gate to include the facility number (photo taken). Licensee and Co-Licensee updated the facility’s Facebook page to include the facility number in the profile banner (photo taken). Licensee and Co-Licensee confirmed with LPA that there is no other advertising signage that they currently have for the facility.
Based on LPA’s observations, interviews conducted where the Licensee and Co-Licensee own admission forgot to include the facility number when advertising their facility to the public, documents obtained, record review, and the investigation, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 3, Section number 102359 (a) Advertisements and License Number, a Type B Citation is cited on the attached deficiency page.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
The Exit interview was conducted with Ofelya Asatryan, Licensee and Paul Ouzounian, Co-Licensee, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
