| On 02/06/2025, Devon Carus & Adrian Risher Licensing Program Analysts (LPAs) conducted a subsequent visit to deliver the findings for an Unusual Incident Report (UIR) reported to the department by the licensee.
A full investigation was conducted by Investigator Veronica Padilla of the Department’s Investigation Bureau regarding a child that was injured at the facility.
The incident occurred at a prior licensed family childcare home licensed by the same provider(1974 94837). However, since the incident the licensee relocated to where she is currently providing care.
Based on the evidence obtained during the investigation, which included interviews with relevant parties, relevant documentation obtained to include medical records there is a preponderance of the evidence, to show that the allegation occurred, therefore the above allegation is substantiated.
A child in care sustained a fractured leg when the licensee did not use care on placing a child in a crib for a nap. Based on the child’s age the crib was not age-appropriate which is a personal rights violation and led to lack of supervision by the licensee when she did not check on the child timely to ensure child had not sustained any injuries and was free from pain. This is an immediate risk to the health & safety risk for children in care and a Type A violation. A $500.00 civil penalty will be assessed for lack of timely care & supervision.
