On 08/24/2021 at 8:00 a.m. Licensing Program Analysts (LPA) Deborah Lowe met with Executive Director Pauline McPeake. The purpose of the visit was an announce pre-licensing inspection at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children were present during this inspection. This is an application for 38 infant children, ages 3 months to 36 months old with a toddler option. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. The fire clearance was approved on 07/14/2021.

Heidy Lara, Alana Spielman, and Chelsea McCants were also present during the visit.

Lobby area has key code entrance. There is another key code entrance to the door leading to the child care area. Program has three buildings, Lobby, Infant/ Toddler, and Preschool. Main Lobby building has a room near the lobby front desk program will use for enrichment/ therapy. This area was not calculated into classroom activity space and will be locked when not in use.

Lobby also has a second story used by Santa Monica Community College for Child Development students to observe the child care. Students of Santa Monica Community College do not have direct access to the child care. Program has camera’s and microphones in each classroom. College students will conduct classroom observations in an upstairs classroom via computer and outdoor observations are available upstairs on an outdoor walkway overseeing the Infant/ toddler playgrounds.

LPA Deborah Lowe toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 8 classrooms (4 infant/ toddler and 4 preschool). Infant/ Toddler Option children will have access to 6 sinks and 4 toilets, and 6 changing tables.

· Toddler Option Room #2= (3.42 x 9.0) = 30.78, (24.58 x 9.75) = 239.66, (13.33 x 22.50) = 299.93, (10.33 x 15.75) = 162.70, (7.17 x 9.17) = 65.75. 30.78 + 239.66 + 299.93 + 162.70 + 65.75 = 798.82

Total Indoor Activity Space Measurements for Toddler Option is: 622.69 + 798.82 = 1,421.51 divided by 35 sq feet = 40 Toddlers Option. Per Health & Safety Code 1596.956(5) The maximum group size, with two teachers, or one fully qualified teacher and one aide, shall not exceed 12 toddlers making total capacity for Toddler Option at 24 Toddlers Option.

Outdoor Play measurements are:

Facility has two outdoor activity areas, one for Infant and a separate gated area for Toddler Option.

Infant outdoor activity space: (32.67 x 48.42) = 1581.88, (18.42 x 22.58) = 416.92. 1581.88 + 416.92 = 1998.80 divided by 75 sq feet = 26 Infants.

Toddler Option outdoor activity space: (33.50 x 75.0) = 2512.50, (22.50 x 23.67) = 532.58, -(7.33 x 3.58) = 26.24, --(3.58 x 1.58) = 5.66. 2512.50 + 532.58 – 26.24 – 5.66 = 3,013.18 divided by 75 sq feet = 40 Toddler Option.

LPA Lowe advised prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant.

High chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding. Bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date.

LPA Lowe advised the infant care center shall not serve honey or corn syrup to any infant.

Infant program with Toddler Component

Licensees serving infants may create a special program component for children who are between 18 months and 36 months of age. The provisions of Sections 101151 through 101239.2 and 101351 through 101439.1 shall apply to infant care centers operating a toddler component in addition to those specified in Health and Safety Code section 1596.956.

The toddler program shall be conducted in areas separate from those used by older of younger children. Plans to alternate use of outdoor play space may be approved to achieve separation.

Indoor/Children’s Area:

LPA Lowe observed all outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard were kept free of obstruction. The child care center was observed be clean, safe, sanitary and in good repair to ensure the safety and well-being of children, employees and visitors. Floors of all rooms have a surface were observed to be safe and clean. A comfortable temperature for children was observed with A/C in each room. Furniture and equipment be was observed to be maintained in good condition, free of sharp, loose or pointed parts. All play equipment and materials used by children was observed to be age-appropriate. LPA Lowe observed individual permanent storage space for his/her clothing, personal belongings and/or bedding. All materials and surfaces accessible to children, including toys, were observed to be free of toxic substances.

The facility has a contract with pest control monthly service provided by Santa Monica Community College to ensure measures to keep the center free of flies, other insects, and rodents. All containers used for storage

of solid wastes, including moveable bins, were observed to have a tightfitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof. All window screens were observed to be in good repair and free of insects, dirt and debris.

Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were observed to be stored in cabinets with child proof latches showing inaccessible to children. Storage areas for poisons shall be locked. The licensee shall maintain the following first-aid supplies in a location accessible to staff but inaccessible to children: LPA Lowe observed a current edition of a first-aid manual, sterile first-aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometer, antiseptic solution in each room.

Every licensed child day care center shall have one or more carbon monoxide detectors in the facility. LPA Lowe was able to hear the test of the carbon monoxide detector. A carbon monoxide detector was observed in the Infant building and one in the Preschool building. Facility was observed to have working telephone service on the premises.

In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following:

The licensee shall be responsible for ensuring that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. Screen questions were observed in the lobby entrance way.

LPA Lowe advised smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). LPA Lowe advised a baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c).

There shall be at least 35 square feet of indoor activity space per child based on the total licensed capacity. Based on the total licensed capacity, one toilet and one handwashing fixture shall be maintained for every 15 children, or fraction thereof.

Restrooms:

All toilets, handwashing and bathing facilities were observed to be maintained in safe and sanitary operating

moisture-resistant material. Crib mattresses shall be: LPA Lowe observed mattresses to be maintained in a safe condition with no exposed foam, batting or coils.

Floor mats or cots that meet the requirements of Section 101239.1(b) shall be provided for all infants who have the ability to climb out of a crib. Cots were observed along with cribs.

LPA Lowe advised each crib, mat or cot shall be occupied by only one infant at a time. Each infant's bedding shall be used for him/her only. Such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant.

Cribs, mats or cots shall be arranged to provide a walkway between cribs, mats or cots to allow staff to reach each infant without having to step over or reach over any other infant.

The calculation of indoor activity space for infants shall not include space designated and used for cribs.

The sleeping area for infants shall be physically separate from the indoor activity space.

Infant Changing Tables

LPA Lowe observed changing tables to have a padded surface no less than one-inch thick and be covered with washable vinyl or plastic. Have raised sides at least three inches high. Be maintained in good repair and safe condition. While in use, be placed within arm's reach of a sink. Not be located in the kitchen/food-preparation area.

Infant Isolation Area

LPA Lowe advised the isolation area shall be equipped with a crib, cot, mat or playpen for each ill infant. Any infant in the isolation area shall be under constant visual observation by a director, an assistant director, a teacher or an aide. A center shall be equipped to isolate and care for any child who becomes ill during the day. The isolation area was observed to be located in an office near the administrator office, facility stated a staff member will stay with an ill child to ensure supervision of children. The isolation area shall be equipped with a cot for each ill child. LPA Lowe advised the isolation area shall not be located in the kitchen area or the general-use toilet area

INFANT PROGRAM – PHYSICAL PLANT

Indoor activity space for infants was observed to be physically separate from space used by children in the child care center components.

Outdoor activity space was observed be physically separate from space used by children in the child care center and school-age child care center components.

The infant care center was observed to be equipped with appropriate furniture and equipment including, but not limited to, cribs, cots or mats; changing tables; and feeding chairs.

Furniture was observed to be maintained in good repair and safe condition. The type, height and size of furniture and equipment was observed to be age appropriate. LPA Lowe advised swings, playpens and all such equipment and furniture shall be assembled or installed according to the manufacturer's instructions, and shall be maintained in good repair and safe condition. No swings were observed within the infant program.

Toys were observed be safe, and did not have sharp points or edges or splinters, or be made of small parts that can be pulled off and swallowed.

LPA advised rattles shall be large enough so that they cannot become lodged in an infant's throat and constructed so that they will not separate into small pieces.

There shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every five infants being toilet trained.

Napping Equipment

A standard size six-year crib was observed meeting the following requirements shall be provided for each infant who is unable to climb out of a crib:

Stacking wall cribs or cribs stacked one on top of another, often referred to as tiered cribs, shall not be permitted.

Cribs shall not limit the ability of staff to see the infant. Crib mattresses shall be: Covered with vinyl or similar

Menus shall be in writing and shall be posted at least one week in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

LPA Lowe advised the licensee shall post signs at the entrance to the child care center that provide the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5.

SIDS & SHAKEN BABY SYNDROME INFORMATION: LPA discussed safe sleep for infants with applicant: Infants must be placed on their backs and must be physically checked every 15 minutes to gauge temperature and ensure they are breathing. Applicant reviewed both items provided and understands the guidance of safe sleep practices. LPA discussed flyer given today (Never Shake a Baby) Applicant reviewed flyer and understands the preventive practices of shaken baby syndrome and abusive head trauma. Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training.

Senate Bill (SB) 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

Senate Bill (SB) 277 New Immunization Requirement: Beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

condition. LPA Lowe advised additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities.

LPA Lowe advised hot water temperature controls shall be maintained to automatically regulate temperature of hot water delivered to plumbing fixtures used by children to attain a hot water temperature of not less than 105 degrees F (40.5 degrees C) and not more than 120 degrees F (48.8 degrees C). Notwithstanding (e) and (e)(1) above, handwashing fixtures shall not be required to deliver hot water.

There shall be one toilet and one handwashing fixture, separate from and in addition to the number of toilets and handwashing fixtures required in (h) above, designated for use by children who are ill, for use by staff, and for emergency use. Isolation area toilet was observed in the office area across from the isolation room.

Outdoor:

Provide a shaded rest area for the children. LPA Lowe observed the outdoor area to be free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. LPA Lowe observed material that absorbs falls.

LPA Lowe advised sandboxes shall be inspected daily and kept free of hazardous foreign materials.

The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence was observed to be at least four feet high.

LPA Lowe advised any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. This shall include, but not be limited to, incinerators, air- conditioning equipment, water heaters, and fuse boxes.

No bodies of water were observed at facility.

There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity.

Health Related Services:

Each classroom had an upper kitchen cabinet marked for medication with an additional box inside marked

medication. Each classroom has a refrigerator to store medication that requires refrigeration.

The licensee has develop and implement a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given. LPA Lowe observed the written plan with child’s authorized representative located in each classroom.

Food Service:

LPA Lowe advised all food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner.

LPA Lowe advised all foods or beverages capable of supporting the rapid and progressive growth of microorganisms that can cause food infections or food intoxications shall be stored in covered containers at 45 degrees F (7.2 degrees C) or less.

LPA Lowe advised pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances shall be stored in areas separate from food supplies.

LPA Lowe advised all kitchen, food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.

LPA Lowe advised the food-preparation area shall not be used for: Children's play activities, unless such activities are part of a supervised food-education program. Full-day programs shall offer a midmorning and a midafternoon snack.

LPA Lowe advised the license shall be posted in a prominent, publicly accessible location in the center.
