LPA attempted to interview children but due to their age and abilities no disclosure was made.
LPA did observe a diaper changing log and schedule posted on the wall located in the day care area. However, based on LPAs observations and interviews which were conducted, and record review the allegation is unsubstantiated.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
An exit interview was conducted with Petra Garza, Licensee in which this report was reviewed with her LPA provided the Licensee with a copy of this report, a Notice of Site Visit (LIC 9213) and Appeal Rights.
LPA advised licensee The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
