On 01/25/2022 at 8:00 am Licensing Program Analyst (LPA) Deborah Lowe met with applicant Tara Perera. The purpose of the visit was a second announce pre-licensing inspection to review for a change in ownership application at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 41children present during this inspection. This is an application for 87 children, ages 2 years to 5 years-old. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. Applicant has a pending application for an Infant License #197494945.

The fire clearance was approved on 10/22/2021.

The 1st pre-licensing visit was conducted on 01/03/2022. LPA Lowe is conducting a 2nd pre-licensing visit to evaluate the facility corrections needed before a license application will be considered.

  • Ensure that all shelving is secured to the wall. LPA Lowe observed shelving units secured to walls.
  • Remove/replace all broken equipment inside of the classroom and outside on the playgrounds. LPA Lowe observed equipment in classrooms and outside to be in good repair.
  • Remove/replace torn sandbags in the outdoor play areas. LPA Lowe observed playgrounds were free of sandbags.
  • Remove/replace changing table in Room 3. LPA observed bathroom in room 3 without a changing table.
  • Remove highchair in Room 3 from restroom. LPA observed bathroom in room 3 to be without highchair.
  • Declutter top of shelves and tables to ensure there are no hazards to children. LPA observed classroom shelves to be free of clutter.
  • Repair peeling/chipped paint and floorboard in Room 3. LPA Lowe observed chipped paint and floorboard in room 3 to have been repaired.

  • All napping mats need to be replaced. LPA observed napping mats replaced with cots. Cots were observed to be in good condition.
  • Remove tape from all fire alarm pull stations and repair if needed. LPA Lowe observed tape from fire alarm pull stations was removed.
  • Ensure that all furniture/play equipment is clean and safe. LPA Lowe observed play equipment to be in good condition.
  • Ensure refrigerator in Room 6 is inaccessible to children so that there are no hazards to children. LPA Lowe observed refrigerator in room 6 to have been replaced. LPA Lowe observed refrigerator to be free of hazards.
  • Make closet in Room 4 inaccessible to children to due to safety hazard inside closet. LPA Lowe observed a child proof door handle cover on closet door making door inaccessible to children.
  • Provide updated facility sketch showing updated classroom numbers, playground names and off limit areas. Applicant stated they will need to send updated sketch to LPA.
  • Install safety latches to teacher’s desk drawers/cabinets containing hazards and sharps to ensure inaccessibility to children. LPA observed safety latches placed on all teacher desk and drawers.
  • Provide safety outlet covers to exposed outlets. LPA observed outlet covers on all unused outlets.
  • All kitchen drawers/cabinets containing hazards and sharps need to be made inaccessible to children. LPA Lowe observed the kitchen to be made inaccessible to children with the use of a locked door.
  • Remove pesticide from kitchen area, ensure poisons are locked and not located in the kitchen area. LPA Lowe observed no poisons or pesticides in the kitchen area.
  • Provide age range for climbing equipment on playground #2. Applicant stated playground was inspected and evaluated on 01/22/2022 and a report will be sent to LPA Lowe once available.
  • Repair/replace padding around the shade structure posts. Padding around shade structure was repaired with blue tape wrapped around padding cover.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children,

including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

The following corrections are still needed before application will be considered, corrections due by 01/31/2022:

  • Provide disaster and mass casualty plan of action.
  • Provide updated facility sketch with room numbers and playground numbers.
  • LPA Lowe observed spring riders in place. LPA Lowe requested a plan to ensure child safety while using playground #1.
  • Provide playground inspection/ evaluation report to LPA.
