25 | On 07/02/24, at 8:30 AM, Licensing Program Analyst (LPA) Brittanee Cleveland and Licensing Program Manager (LPM) Raul Navarro conducted an unannounced case management inspection at the facility stated above. LPA and LPM toured facility with the guidance of Annel Chavez, assistant director. There were 25 children present with 6 staff during the inspection.
During tour of the facility, LPA and LPM were joined by Director, Tara Perera, at 9:35 AM. LPA and LPM toured preschool classrooms, where teacher’s names and number of children were obtained. While touring the play areas, LPA and LPM observed equipment with breaks and have been worn down. Director was advised by LPM and LPA to replace the equipment.
Director informed LPA and LPM that due to the temporary inactive status, new equipment was purchased and has not been delivered.
LPA and LPM were able to conduct reviews of staff and children files. After review, all files were cleared and accurate.
All kitchen areas/food preparation areas and food storage areas are kept clean and are free of any hazards. LPA observed the kitchen to be safe and sanitary.
LPA will deliver Technical Violations (TV) to facility.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
A copy of the report and appeals rights were provided to the Director. An exit interview was conducted and report was reviewed with Tara Perera, Director. |
