their own diapers, wipes, and ointment and are changed when they arrive, through out the day and before they go home. Per Director, if a child gets a diaper rash while in care, the parents are notified.

Staff interviewed stated children that wear diapers are changed when they first arrive to the center, then every 2 hours or as needed, and before they go home . Staff stated if a child gets a diaper rash in care they notify parents at the time of pick up.

Staff interviewed stated they always count the children before going outside and coming back inside from the playground. Staff stated their are generally 2 classrooms outside at a time with 2 staff members and they survey opposite sides of the yard. Staff stated if a child gets an injury at the center they will complete a boo boo report and notify the parent at the time of pick up, or if they get a serious injury the Director will notify the parent and send a picture.

Parents interviewed did present concerns regarding supervision but not regarding supervision that results in child on child injury. Parents interviewed did not present concerns regarding diapering needs.

Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
