Infant program is located on the property of Shomrei Torah Synagogue in the second building to the north of the drive way after you pull in from Valley Circle Boulevard. Infant classrooms are located on the bottom floor. Second floor is occupied by an elementary/ middle school program. There is a total of four buildings on property, one used for Synagogue and two other buildings used for 3^rd party educational programs.

LPA Lowe toured and measured the on-limits areas and classrooms located at the facility. The facility has a total of 4 classrooms to be used for infants, rooms# 9, 10, 11, and 12. Room #9 will be used as a crib room and will not be calculated into overall capacity measurements. Infants will have access to 4 sinks. Fire extinguisher was observed with a service date of 12/21/2021.

Classroom measurements are:

· Room #10 = (19.25 x 25.75) = 495.69

· Room #11 = (23.33 x 23.25) = 542.42 – encumbered space (3.25 x 3.17) = 10.30 – (5.33 x 2.05) = 10.93 – (3.75 x 2.17) = 8.14. Total activity space: 542.42 – 10.30 – 10.93 – 8.14 = 513.05

· Room #12 = (23.00 x 23.42) = 538.66 – encumbered space (2.05 x 5.33) = 10.93 – (1.75 x 3.92) = 6.86. Total activity space: 538.66 – 10.93 – 6.86 = 520.87

· Room #9 crib room = (19.05 x 25.25) = 481.01. Room 9 was observed to have 12 cribs.

Outdoor Play measurements are:

Infant outdoor area is located behind rooms 11 & 12 on the North side of building.

(87.33 x 31.25) = 2,729.06 divided by 75 square feet = 36 infants. Outdoor measurement includes play structure, applicants will need to verify the appropriate age usage for the play structure.

Indoor/Children’s Area:

Indoor activity space for infants was observed to be physically separate from space used by children in the child care center and school-age child care center components.

All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard were observed to be free of obstruction. The child care center was observed be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Floors of all rooms have a surface that is safe and clean. A comfortable temperature for children was observed. The infant care center was observed to be equipped with appropriate furniture and equipment including, but not limited to, cribs, mats; changing tables; and feeding chairs. Tables and chairs were observed to be meet the needs of the children. Furniture shall be maintained in good repair and safe condition.

The type, height and size of furniture and equipment was observed to be age appropriate. LPA advised swings, playpens and all such equipment and furniture shall be assembled or installed according to the manufacturer's instructions, and shall be maintained in good repair and safe condition. No swings or playpens were observed during visit.

LPA advised toys shall be safe, and shall not have sharp points or edges or splinters, or be made of small parts that can be pulled off and swallowed. Rattles shall be large enough so that they cannot become lodged in an infant's throat and constructed so that they will not separate into small pieces.

LPA Lowe observed individual permanent storage space for infants clothing, personal belongings and/or bedding. All materials and surfaces accessible to children, including toys, were observed to be free of toxic

Drinking water from a Brita container was observed to be readily available both indoors and in the outdoor activity area. The applicant stated the Synagogue has a pest control service to assist in keeping the center free of flies, other insects, and rodents. All containers used for storage of solid wastes, including moveable bins, were observed to have a tightfitting cover that is kept on; observed in good repair; and to be leakproof and rodent-proof. No fireplaces and open-faced heaters were observed at facility. All window screens were observed to be in good repair and free of insects, dirt and debris.

LPA advised disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. Applicants stated main storage of cleaning solutions shall be stored outside of classrooms and inaccessible to children. LPA advised storage areas for poisons shall be locked. The licensee shall maintain the following first-aid supplies in a location accessible to staff but inaccessible to children: LPA observed a current edition of a first-aid manual, sterile first-aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometer, and antiseptic solution.

LPA Advised any infant in the isolation area shall be under constant visual observation by a director, an assistant director, a teacher or an aide. Applicants stated isolation area will be located in the directors office.

The isolation area shall be equipped with a mat each ill infant. LPA advised the isolation area shall not be located in the kitchen area or the general-use toilet area.

Facility was observed with a combination smoke and carbon monoxide detectors in each classroom and crib room. Applicants tested detectors and LPA was able to hear a successful test. Facility was observed to have working telephone service on the premises.

Applicants stated facility will use the Brightwheel App as a sign in/out method for families.

LPA Lowe reviewed with applicant the facilities policy in ensuring infants with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. Applicant provided a wellness check plan to be completed daily upon drop off.

LPA advised smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). LPA advised a baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c).

Napping:

LPA Lowe observed 12 standard size six-year crib for each infant who is unable to climb out of a crib: LPA advised Stacking wall cribs or cribs stacked one on top of another, often referred to as tiered cribs, shall not be permitted. Cribs were observed to not limit the ability of staff to see the infant. Crib mattresses were observed to be covered with vinyl material. Crib mattresses were observed to be maintained in a safe condition with no exposed foam, batting or coils.

LPA observed floor mats that meet the requirements of Section 101239.1(b) to be provided for all infants who have the ability to climb out of a crib. LPA advised each crib, mat or cot shall be occupied by only one infant at a time. Each infant's bedding shall be used for him/her only. Such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant.

LPA advised the calculation of indoor activity space for infants shall not include space designated and used for cribs. The sleeping area for infants was observed to be physically separate from the indoor activity space in room #9.

Mats used for napping were observed to be maintained in a safe condition. Constructed of foam at least 3/4 inch thick. Covered with vinyl or similar material that can be wiped. Marked or colored so that the sleeping side can be distinguished from the floor side. Wiped with a detergent/disinfectant weekly or when soiled or wet.

LPA advised mats shall be maintained in a safe condition with no exposed foam. LPA advised each mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child. Applicants stated infants will provide their own bedding.

The facility was observed to have a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.

Infant Changing Tables

Have a padded surface no less than one-inch thick and be covered with washable vinyl or plastic. Have

Restrooms:

All toilets, handwashing and bathing facilities were observed to be maintained in safe and sanitary operating condition. Additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities.

Hot water temperature controls were observed be maintained to automatically regulate temperature of hot water delivered to plumbing fixtures used by children to attain a hot water temperature of not less than 105 degrees F (40.5 degrees C) and not more than 120 degrees F (48.8 degrees C).

LPA advised notwithstanding (e) and (e)(1) above, handwashing fixtures shall not be required to deliver hot water.

There shall be one toilet and one handwashing fixture, separate from and in addition to the number of toilets and handwashing fixtures required in (h) above, designated for use by children who are ill, for use by staff, and for emergency use. This toilet and handwashing fixture shall be conveniently located in relation to the isolation area. Staff bathroom located near the office will be used as an isolation restroom for an ill child.

LPA advised common towels or washcloths are prohibited.

LPA advised there shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every five infants being toilet trained. The facility was observed with 1 child size toilet, 2 adult size toilets. Applicants will provide potty chairs for potty training infants.

Outdoor:

Outdoor activity space was observed be physically separate from space used by children in the child care center and school-age child care center components.

LPA observed a shaded rest area for the children. Outdoor activity area was observed to be free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.

LPA advised sandboxes shall be inspected daily and kept free of hazardous foreign materials.

The playground was observed to be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence was observed be at least four feet high.

LPA advised any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. This shall include, but not be limited to, incinerators, air- conditioning equipment, water heaters, and fuse boxes.

LPA did not observed any pools or other bodies of water at facility.

There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity.

Health Related Services:

LPA advised medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration. Applicant stated all medications will be stored in a cabinet located in the office, medications that require refrigeration will located in small office refrigerator or in classroom #11, refrigerator is located in a child proof locked cabinet inaccessible to children.

LPA and applicants discussed facilities written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Food Service:

LPA advised prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. High chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding.

LPA advised bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. The infant care center shall not serve honey or corn syrup to any infant.

LPA advised all foods or beverages capable of supporting the rapid and progressive growth of microorganisms that can cause food infections or food intoxications shall be stored in covered containers at 45 degrees F (7.2 degrees C) or less. LPA observed a thermometer in the refrigerator /freezer.

LPA advised pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.

LPA advised soaps, detergents, cleaning compounds or similar substances shall be stored in areas separate from food supplies.

LPA advised all food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.

The food-preparation area shall not be used for: Children's play activities, unless such activities are part of a supervised food-education program. Full-day programs shall offer a midmorning and a midafternoon snack.

Full-day programs shall ensure that each child has a lunch. Half-day programs shall offer a midmorning or midafternoon snack.

Postings:

LPA advised the license shall be posted in a prominent, publicly accessible location in the center.

LPA Lowe observed the following postings: LIC 613A (9/96) in a prominent, PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster, LIC 610 which provides the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5, and LIC 9148 Earthquake Emergency Preparedness plan publicly accessible area in the center.

LPA reviewed the facilities disaster and mass casualty plan of action. The plan was writing and was observed to be readily available. The mass casualty plan is in connection with Shomrei Torah Synagogue disaster and mass casualty plan.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

· Arrange room #11 and #12 to ensure changing table is within arm’s distance of a sink

· The gate outside of room #12 to outdoor play is in need of repair

· Secure green chain link fence covering in outdoor play area near sandbox

An exit interview was conducted with the above items discussed and a copy of this report was provided to the Applicant Jennifer Pearlman and Eliza Enbom. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
