On 12/18/2024, Licensing Program Analyst (LPA) Lisa Clayton made an unannounced visit to the Sall Family Child Care Home to conduct the Required 3-year inspection. LPA was greeted by Licensee Sokhna Sall. LPA Clayton observed 6 children being supervised by Licensee and a volunteering assistant. Per Licensee the hours of operation are Monday through Friday 8am – 5pm. Licensee provides snacks, and water.

LPA toured the entire home inside and outside for a Health and Safety inspection. The home is neat and clean and has ventilation for safety and comfort. The home has working telephone and LPA Clayton confirmed the number is (213) 712 0727.

LPA Clayton confirmed that the single-family home consists of the following: living room, kitchen/dining room, 3 bedrooms, 1 bathroom, detached converted garage with an attached storage room, and fenced front and back yards.

The ON LIMIT AREAS are as follows: living room (day care activity room), bathroom, bedroom #3 (activity/napping room) converted garage for day care activities only (no eating, toileting or sleeping) and fenced backyard. The ISOLATION AREA will be bedroom #3.

The OFF-LIMIT AREAS are as follows: the kitchen/dining room, bedroom #1, bedroom #2 and the garage storage room, all of which are inaccessible to children in care by child safety gates and/or closed and/or locked doors and visual supervision.

Furniture and equipment are in good condition and free of sharp, loose, or pointed parts.

There are no pools, ponds or any other bodies of water on the premises. There are no firearms or ammunition on the property.

Based on LPA Clayton observation of matches, lighters and bleach wipes in the bathroom accessible and within reach of children in care, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to children in care.

Licensee will ensure that any and all poisons, detergents/cleaning compounds, medication and hazardous items that can pose a danger to children are made inaccessible to children in care.

Based on LPA Claytons record review of Guardian and both the licensee and the volunteer assistants admission that she was fingerprinted only yesterday (clearance not yet recieved) but had been working at the FCCH since August 13, 2024 the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

The home has a fully charged 2A10BC fire extinguisher in the kitchen, and working carbon monoxide detector and working smoke detector in the hallway.

While inspecting the bathroom LPA Clayton observed a book of matches and a lighter in the medicine cabinet, and bleach wipes and a fireplace lighter in the bathtub. LPA Clayton reminded Licensee that any poisons, detergents/cleaning compounds, medication and hazardous items that can pose a danger to children are to be made inaccessible to children in care.
Licensee stated that she conducted her last disaster drill in January 2024. LPA Clayton reminded licensee that a family child care is required to conduct fire drills and disaster drills at least once every six months and the licensee shall document the drills, including the date and time of each drill, and keep the documentation at the family child care home.

LPA Clayton reviewed 5 children’s files which contain current contact information for authorized representatives and/or relatives who can assume responsibility for the child, signed Parent’s Rights, current immunization records, and a signed Consent for Emergency Medical Treatment.
Licensee has current CPR/First Aid and Mandated Reporter certificates that expires July 2025. Licensee has immunizations records that are in compliance with immunizations required for childcare providers.

LPA Claytons review of Guardian indicated that the volunteer assistant present in the home has not been granted a fingerprint clearance as of today December 18, 2024. In addition, a review of the assistant file is as follows:

· Incomplete CPR (missing First Aid)

· Incomplete Mandated Reporter training (missing Child Care Providers training)

· Unable to verify MMR Immunizations (the records are in French, LPA instructed that she have them translated and transcribed prior to her return to the FCCH)

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A Type A Violation and a Civil penalty of 100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated

Licensee stated that the FCCH does not provide care to children under 18 months of age. LPA Clayton and licensee reviewed the Safe Sleep regulations and LPA Clayton reminded license to check the Safe Sleep webpage on the Child Care website https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and­resources/safesleep as an additional resource.

LPA Clayton informed applicant of the importance of checking for recalled infant devices on the United States Consumer Products Safety Commission (CPSC) website at https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on the purchased equipment.

Incidental Medical Services (IMS) are currently being provided. LPA Clayton instructed licensee that all medications are to be stored in a locked box, inaccessible to children in care. LPA Clayton provided Licensee with an IMS Plan sample and instructed her to submit the completed plan to the office immediately upon completion. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.



LPA reminded Licensee of the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&R) throughout California.

During the exit interview, Licensee confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO profile in FAS.

Per Title 22 Regulations and Health and Safety Codes, Deficiencies and Technical Violations were cited today (see LIC 809D and LIC 9102).

Licensee will provide parents of currently enrolled children and parents of children enrolled over the next 12 months copies of the Licensing Report containing the Type A violations and maintain a signed LIC 9224 in the children’s files.

Licensee will submit to the department a photo of the medication in a locked box in an area inaccessible to children no later than Friday December 19, 2024.

Licensee will submit a completed IMS to the department no later than January 2, 2025 or before the child returns to the FCCH.

An exit interview was conducted, a copy of this report was read and provided to licensee and will be made available to the public upon request.

LPA Clayton posted a Notice of Site Visit which is required to be posted for 30 days.
