On 5/14/24, at 8AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an announced follow up capacity increase / Annual Random Inspection. The purpose of this visit is to ensure the corrections requested during the initial visit conducted on 4/15/24 had been completed by the Licensee. Licensee has submitted a capacity increase application for a large family child care license with up to 14 children - ages 1 month - 13 years of age. The facility’s hours of operation are 8AM - 11:59PM, Monday - Friday. Upon arrival, LPA met with Licensee / Azatuhi Avetisyan, who guided LPA on a tour of the inside and outside of the home. LPA observed 6 children in care of which 2 are infants.

During the initial capacity increase / Annual Random Inspection, corrections were requested and the items listed below remain in need of correction:

· During the visit conducted on 4/15/24, LPA observed large bricks and trees/shrubs located in the play yard, which can be a hazard to children. Per Licensee, fencing will be installed separating the play area from the bricks and trees/shrubs. During today's visit, LPA observed the front yard / play area (located on top of the artificial grass) was not fully enclosed by the small fencing, making the large bricks and trees/shrubs accessible to children. Per Licensee, the missing parts of the fence will be delivered within the next few weeks and will be installed upon delivery.

· During the visit conducted on 4/15/24, LPA observed a "Gorilla Skyline Lookout Outdoor 3 Story Playset" located in the play yard which is not age appropriate for children under the age of three (3) years old. Per Licensee, perimeter fencing will be installed around the "Gorilla Skyline Lookout Outdoor Playset", ensuring it is inaccessible to children under the age of three (3) years old. During today's visit, LPA did not observe perimeter fencing installed around the "Gorilla Skyline Lookout Outdoor Playset", ensuring it is inaccessible to children under the age of three (3) years old.

The Licensee will ensure that corrections are made by 5/31/24 and notify CCL. Licensee will submit pictures/videos of the small fencing once it has been completely enclosed in the front yard / play area (on top of the artificial grass), preventing children from having access to the large bricks and trees/shrubs. Licensee will also submit pictures of the "Gorilla Skyline Lookout Outdoor Playset" making it inaccessible to children under the age of 3 years of age.

Exit interview conducted and report was reviewed with the Licensee / Azatuhi Avetisyan.

A copy of this report was provided to the Licensee. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
