25 | On 08/22/2024 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes.
LPA Laureano met with Angela Pina, Licensee, and toured the home both indoors and outdoors.
LPA Laureano reviewed children’s file and staff files and observed files to be incomplete. Mandated Reporter Training certificate was not valid for both Licensee’s and Assistant K. Pina. LPA observed 4 out of 7 children’s files incomplete, missing the LIC 627 Consent for Emergency Medical Treatment.
2 Type B deficiencies were cited during today’s inspection in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1 and California Health and Safety Code. Please see LIC 809D for reference.
Upon on receipt of this report, the Licensee shall post the Notice of Site Visit.
The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
An exit interview was conducted, and report was reviewed with Licensee. A copy of this report and appeal rights were discussed and left with the Licensee, whose signature on this form confirm receipt of these documents.
