On 1/24/2024 Licensing Program Analyst (LPA) Jillinda Chandler, made an announced visit to the Leap and Bound Academy Medical Center, located at 23805 Hawthorne Blvd. Ste. 101, Torrance, CA. 90505 for the purpose of conducting a pre-licensing inspection for adding an infant component (197495322) to an existing preschool component (197418344). LPA met with Charles Thacker (Applicant/ Owner) and Dejahnae Alexander (director). A tour of the facility was provided and all areas, according to the provided facility sketch, were inspected for Health and Safety compliance, per Title 22.
The facility is located on the lower level of a multi-business suite. There is an approved fire clearance on file conducted by Inspector Krieger of the Torrance Fire Department, Fire Prevention Unit.
The applicant is requesting a license for 24 infants; ages 0-24 months .The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m.

LPA observed the following of the indoor activity space:


Age appropriate furniture and equipment was observed in good repair
A variety of age-appropriate equipment, toys that provide and encourage Auditory stimulation, Visual stimulation, Tactile stimulation, and Manipulative skills.
LPA observed 8 cribs and mats for napping in good condition. LPA observed that the portable barrier around the crib did not completely make this area inaccessible to indoor activities; Per Title 22, Section 101438.3 (c)(1) The calculation of indoor activity space for infants shall not include space designated and used for cribs, (1)The sleeping area for infants shall be physically separate from the indoor activity space. The applicant shall devise a plan to make the crib area inaccessible to indoor activities.

Fire extinguishers last inspected on 11/20/2023 and carbon monoxide detectors were observed. LPA observed one changing table within arms reach of a sink and padding at least 3 inches thick and raised sides. Waste bins were observed with tight fitting lids, for soiled beddings and materials. First aid kits were available with the required essentials: scissors, bandages, tweezers, and thermometer ,Adequate lighting was observed
No Fireplaces or open face heaters were observed, Disinfectants and cleaning solutions and other toxins or poisons were made inaccessible to children, placed in locked storage room. The directors office and and 1 designated toilet area shall be used for Isolation of ill children. The facility has enough toileting areas to designate one to ill children, the director was informed that this toilet area must not be used by other children in care during isolation.The classrooms were clean and in good repair.

FOOD SERVICES
Parents will provide formulas and baby food, foods shall be labeled and stored accordingly. Unused formulas and food shall be discarded immediately after feeding.
filtered drinking water will be provided. Refrigerator with a thermostat was observed for storage of infant formulas and foods capable of contamination. LPA observed 1 low feeding/activity table with 2 feeding seats. The provider will provide meals for the older infants, menus were posted for review.

RESTROOMS

THERE WERE:


3 sinks, 2 toilets, 2 urinals and 1 changing tables, within arms reach of a sink.

Measurements for the indoor space were:
847.65 square feet, which will accommodate the requested capacity of 24 infant children.

OUTDOOR ACTIVITY SPACE
Age appropriate toys and equipment were observed in fair repair.
The play yard was enclosed and separate from other components at the facility, with a 4 inch or higher gate. Resilient cushioning was observed in good repair under fall zones.
LPA did not observe in place shading, applicant had a tent that will be but in place for shade, the built in retention wall will be used for resting. LPA did not observe any hazardous conditions during todays inspection.

Measurements for the outdoor activity area were 816.8 for a total of 10 children, which will not accommodate the indoor capacity of 24 children. The applicant has requested a waiver of Title 22, Section 101238.2(a) - Outdoor Activity Space

Based on today’s inspection the facility shall be recommended for a capacity of 24 infants based on the indoor measurements and pending approval of requested waiver.

The following recommendations were made during todays inspection and shall be provided by prior to licensure, and no later than 2/24/2024:
  • The applicant shall devise a plan to make crib area inaccessible to indoor activity


Child Care Centers applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified, of any recalls on their purchased equipment.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Exit interview conducted and report was reviewed with the applicant Charles Thacker (Applicant) and Dejahnae Alexander (director).
