On 8/14/2025 Licensing Program Analyst (LPA) Jillinda Chandler and Doris Whitmore made an announced visit to Champions for Progress dba JAM Child Developement Center located at 333 Florence Avenue, Inglewood, CA. 90301.The purpose of todays visit was to conduct a pre-licensing inspection. LPAs met with Terrie McCaskill(applicant) and Edegene Warner (director)who provided a tour of the facility. The applicant is requesting a license with a capacity of 30 pre-school children, ages 2 years – until entry into first grade;15 toddlers ages 18 -36 months and 12 infants ages 0 - 24 months. There was one room for infants (#1 infants); one room for toddlers (#2 one year olds) and 3 classrooms dedicated to the preschool: room #3 (2 year olds), room # 4 (three year olds) and room # 5 (4 year olds). Daycare days and hours will be Monday – Friday, 7:00 A.M.– 6:00 P.M. There is an approved fire clearance on file conducted by Inspector Micheal Judkins of the L.A County Fire Department Fire Prevention Bureau.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed of the:

INDOOR ACTIVITY SPACE

Fire extinguishers were 2AB10C or larger, last serviced 2/26/2025.

Carbon monoxide detector were observed in at the entrance and above classroom 3 (2 year olds).

First aid kits were available (located in the kitchen)with the required following essentials: bandages, tweezers, ointments, and manuals.

Age-appropriate furniture and equipment was observed in good repair.

Cots were observed for napping, LPAs observed 4 cribs for infants that are unable to nap on cots. Applicant was advised that sheets must be tight fitting and there shall be no blankets, bumpers or anything hang over the cribs. LPA did not observe any drop down sides and 3 cribs were fitted with emergency evacuation wheels.

LPAs observed 4 wide based high chair for feeding, applicant was informed that children are only to be in high chairs while eating.

LPA observed combination of infant toys that provided and encouraged: Auditory stimulation,Visual stimulation,Tactile stimulation and Manipulative skills.

Cubbies were observed for children’s belongings.

Drinking water will be provided through filtered water dispensers.

The facility has central heating and cooling.

Windows were in good repair free of chipping paint, dirt, insects, or debris.

Adequate lighting was observed.

No cameras were observed during todays inspection.

The classrooms were clean in good repair.

Trash cans used for solid waste were observed with tight fitting lids inside of the classroom, trash cans in restrooms need tight fitting lids.

No Fireplaces or open face heaters were observed.

Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children in care.

The director office will be used for isolation area for ill children and ill children will use the children's restroom with sanitizing and disinfecting after each use.

The facility was equipped with a working telephone in the directors office

Parents and authorized adults will sign children in and out, an electronic devise. Applicant was advised that the sign in sheets shall be readily available for printing at the request of any department representative and maintained for a minimum of 30 days

The required postings were posted in a prominent area for viewing.

Measurements for the indoor activity space were:


infants - 319.29 = 9 children, Toddler - 411.58 = 11 children, and pre-school -1090.69 = 31 children which will not accommodate the applicant’s request for the toddler and infant capacity.

FOOD SERVICE:

Meals will be will be provided by the center. Weekly menus were posted for review.

LPA observed a kitchen, with a sink, storage for foods and a refrigerator without a thermometer, foods and toxins or chemicals were stored separately, and LPA did not observe any expired or contaminated foods.

Center shall devise an Incidental Medical Service (IMS) plan to provide to parents of children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing G-tube feeding. IMS was discussed with the applicant.

RESTROOMS

THERE WERE:

5 sinks and 6 toilets available for children use, and two potty chairs. LPAs observed 3 changing tables within reach of sinks.

Trash cans with tight fitting lids shall be added to restrooms

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered hot and cold water. The hot water will be shut off.

OUTDOOR ACTIVITY SPACE

Age-appropriate toys and equipment were observed in fair condition.

The play yard was gated with a 4 foot or higher gate.

Resilient cushioning was observed in good condition under climbing apparatus.

LPA observed sail shades for shade and benches for resting. Preschool yard needs privacy covering and make the gapping at the north end of the gate at the bottom is inaccessible to children Shading tents must be anchored

Water was readily available for an outdoor water source; children use their personal water bottles for drinking.

LPAs did not observe any bodies of water on the premises

Measurements for the combined outdoor activity area were infant/toddlers yard 824.94 = 11 and preschool - 2168.22 = 28 children ( the front yard can accommodate 23 children and the indoor play area will accommodate 5 children), which will not accommodate the requested capacity. The applicant shall request a waiver for both outdoor spaces.


Based on today’s inspection the facility shall be recommended for a capacity of 9 infants, 11 toddler and 30 pre-school children determined by the measurements of the indoor areas.

An exit interview was conducted and a copy of this report was discussed and provided to Terrie McCaskill (applicant).
