On 08/29/2024 Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes.

LPA toured the facility and observed 93 children in care with 13 staff members providing care and supervision.



Upon arrival, LPA met with Assistant Director Aisha King and discussed the purpose of the visit. LPA observed that only two Notice of Site Visit were posted on the Parent Board. LPA inquired whether the Parent Board had been relocated and Assistant Director King informed her that the documents had just been taken down last night and would be relocated into the Director’s office. LPA reminded Assistant Director King that required documents shall be posted in a prominent, publicly accessible location in the center.

Shortly after at approximately 09:13am Director Demi Lara arrived at the facility and asked LPA what documents needed to be posted. LPA provided Director Lara form LIC125 Entrance Checklist – Child Care Centers. Director Lara promptly started placing required documentation in a publicly accessible location. LPA on previous visits has seen required documentation posted in the facility.

During today’s inspection it was revealed that staff do not clean classrooms floors after every snack or meal and continue with the daily activities for the children in care. At approximately 08:34am LPA entered the classrooms prior to any children being in them an observed food crumbs in the classroom floors. At approximately 09:49am LPA began a second inspection of the facility and enter classroom Prepper B which at that time were no children or staff present.

LPA then observed additional food crumbs on the floors and at 09:51am LPA also observed a milk spill on the floor next to the tables designated for eating. At 10:01am LPA enter classroom Preschool 1 and observed food crumbs on the floors which additionally had been stepped on by the children. At approximately 10:06am LPA observed an opened lollipop laying on top of a 5 cubic storage compartment in the classroom, at 10:07am LPA observed a child in care with the lollipop in her mouth. At 10:27am LPA entered classroom Twaddler B and observed an eaten food item and crumbs on the bottom shelf of a 5 cubic storage compartment. Around 11:18am from the outside window in the hallway noticed that the milk spill in classroom Prepper B had not been cleaned up from the floor.

Based on observation, the facility did not comply with the Buildings and Grounds section 101238(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

There was one deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information.



An exit interview was conducted, and Plan of Correction was reviewed and developed with the Director. A copy of this report and appeal rights were discussed and left with the Director, whose signature on this form confirm receipt of these documents. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Director agrees to develop and implement
a plan to make sure that they are compling with maintaining the classrooms clean and sanitary.
Director will submit a developed cleaning plan in writing and a cleaning classroom log to LPA via email by the POC due date.

Based on observation, the facility did not comply with the section cited above, LPA observations done on 6/26/2024 and 8/29/2024t it was found that the facility staff do not keep classrooms clean and sanitary, which poses/posed a potential health, safety or personal rights risk to persons in care.
