On 3/19/2025, Licensing Program Analyst (LPA) Loyce Phillips arrived at the facility to conduct an unannounced visit for the purpose of delivering the findings on the above allegation and to complete staff interviews for this complaint. Upon arrival LPA met with Director, Telma Rojas and toured the facility. LPA observed 72 children present with 21 staff members.

LPA conducted a full investigation that included 3 visits to the facility, interviews with parents and staff, a staff file review and documenting LPA’s observations. LPA interviewed 7 staff members. 5 out 7 staff members stated they have witnessed several staff members engaging in arguments in front of children and other staff members on numerous occasions.

3 staff members stated a child was hit by a half door located in the classroom. S1 stated she hit the child on the head when she open the door. 1 staff member who was present in the classroom stated she only heard the child being by the door. S3 stated she witness the incident and provided ice and comforted the child. Staff were also interviewed about their qualifications; all staff stated they have proper qualifications to work with children in the classrooms. During staff file review, LPA reviewed 11 staff files and observed staff 1 did not have infant and toddler units. On 1/22/2025 and 2/10/2025, LPA observed S1 acting as the lead teacher and supervising 2 staff members in the infant room.

Staff were asked about children’s illnesses; staff disclosed the children are checked first thing in the morning before entering the classrooms and if a child gets sick throughout the day, the office staff is notified, and parents are called to pick their children up. S3 stated she does not do health checks on children in the morning. S3 also stated children are allowed to stay in the facility while they are sick and not feeling well, with the other children. On 2/10/2025, LPA observed children arriving to Quest, Destiny 1 and Harmony 2 classrooms without proper health checks conducted by staff. LPA interviewed 5 parents, and 4 parents stated staff do not do health checks in the morning and do not monitoring children illness. Parents stated they have witness other children arriving to the facility with running noses.

Based on the evidence obtained and interviews conducted, the allegations that staff hit child in care; staff engaged in an argument in front of children in care; staff do not have proper qualifications, and staff did not monitor child's illness while in care are all deemed substantiated. A Substantiated finding means that the allegation is valid because the preponderance of evidence standard has been met.

Deficiencies are cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety Codes.

The notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will results in a civil penalty of 100.00.

Exit interview conducted, report and appeals rights were discussed and provided to Director, Telma Rojas.

Director will have a training with teachers, support staff and adminstrative staff every month for the next six months to discuss Personal Rights. Director will send a summary of what was discussed in the training by POC date.
