On 08/06/2025 at 10:10a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit to follow up on conducting a Case Management Inspection due to an incident that occurred on 07/31/2025. LPA met with Telma Rojas, Center Director and informed the nature of the visit to continue with interviews. At the time of the visit there was children and 19 teacher’s and 68 children.

(S1) stated I I was standing in the hallway with my children. I had to take my children to the bathroom. A parent was standing at the door trying to open it. The children were looking. The parent was yelling loud. The parent was saying let me in, they need to get rid of you. I was more concerned of trying to keep my kids calm. I took my children back to the classroom to try and distract them because they were looking frightened. The parent got on the phone and she was talking to someone. The parent walked out. I did not see anything else. She was shouting because she was angry about it.


( S2) stated The parent called over the phone to ask a couple of questions. ( S3) transferred the call so she can talk to the teacher. A few minutes after that the doorbell rung ( S3) went to answer the door. I saw ( S3) walking towards the office. The parent was walking behind her saying something loud. ( S3) came in and said do not open the door. ( S3) stated when she opened the door the mom started talking and screaming. Ms. ( S3) asked her to calm down and go home. When ( S3) closed the door, she pulled the door to open it. ( S2) said that’s what happened. We were sitting in the office and the parent was screaming saying I want to the speak to the owner Deidre. Someone needs to call her needs to call her. The parent stated that she was not going to leave until someone spoke to her. The parent stated, I don’t know who she thinks she is. I am not leaving until I talk to Deidre. I kept working on the computer she was still outside doing the same thing. The phone rang she calling the office phone. She said the same thing. I know you guys see me out here. I am not leaving until I speak to Ms. Deidre. ( S3) told me to tell her that I was going go outside the office to speak to her. I hung up. ( S3) told me to tell her I advise her to go out the school. Ms. Deidre will not be here, and Ms. Deidre will call her when we give her the message.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

When I opened the door, I said Excuse me can I talk to you. She said wait a minute I need to go check on my kids. There was another teacher in the hallway waiting to use the restroom. The parent came out the door and went to her kid’s classroom. The parent was screaming and yelling. I don’t know what the conversation was. I waited outside the office for her. When she came back in, I asked her if I could talk to her. She said sure. I won’t do here because there is a teacher and kids are here. She said sure she followed me to the front door. I told her Ms. Deidre is not here, I will give her the message to call you back. She said I don’t know what’s going on here. I need to speak to the owner. That lady ( S3) the director is disrespecting me and treating me bad, she closed the door in my face. I called to ask her a question about my kids, and she transferred me to the teachers. I wanted to ask her a question. I did not want to be transferred. She said she is rude and disrespectful Everytime. I try to speak to her she treats me the same way. I am pretty sure she is mistreating my children. She said I am a parent and I know my rights. I asked to see the camera and she did not let me do that. I already went down to the courthouse. I have a right to see the video. Something needs to be done. She said when is Ms. Deidre is going to get here. I need to speak to her she soon as possible. I told her we will relay her the message to Deidre so that we can call her soon as possible, The parent said again she is rude, disrespectful and she is racist. She is discriminating me and my family.

There was a parent that walked in with her child and I kind of kept quiet. I told her again I will let me know Ms. Deidre know you would like to speak to her. Ms. Deidre is not coming in. The parent said we love the school. You have been very nice to me and helpful. My kid love coming here. Its her that I have a problem with. I also like Rebecca when she was here. All of the teachers here are nice. I said ok I will let Ms. Deidre know she will call you back. Yes please have her to call me , I need to speak to her soon as possible because something needs to be done about the situation. The parent said if she wants to call me I will have my phone handy or if she wants to meet with me we can meet outside. There is a lot of things going on with the school with the director. She is going to ruin the school and the school is going to go down, I said ok. The parents stated that it has to be this week not next week. I won’t be available because I am going out of town.

( S3) stated that At around 9:30a.m.in the morning, the parent she was at the door, and she had already dropped off and checked her kids in. She rung the bell, and I walked over to the door. When I opened the door, I realized that it was the mom that just ended a conversation with me over the phone.

The conversation was disrespectful. She continued to be disrespectful at the door to me. She was disrespectful in an aggressive manner. I asked her in such a way in the director that represents the well being of everyone that is in the facility. I preceded to close the door. As I was closing the door she forced her way into the door from her end. She came in. At that moment I became fearful of my well-being and I walked away from her. I made my way into the office. I closed the door. The mom remained there at the door. The mom said disrespectful inappropriate things about me loudly. At that moment I was fearing for my own safety. I was feeling intimidated by her. I asked ( S2) to go out of the office in the hallway with the parent To tell her that she needed to leave the facility and that if she did not we would have to call the police. At that moment. ( S2) and the mom were outside the door and the mom continued to speak loudly exposing the classrooms and parents walking to the disrespectful things out loud. At that moment Ms. Violet was fearing for her well being and became more concerned about the children who were at the restroom. At that point (S2) asked, if the children could go outside. During that time, I did call my supervisor Ms. Deidre to let her know what was unfolding. She then said that I could tell the parent that I was going to call the police. I did hang up with Ms. Deidre and I did waited however at the same time. I did dialed Hawthorne Police Number. I got no answer. There was a voice mail. I hung up from that phone call. Soon after that Ms. Violet did come back in the office. Ms., Violet told me the mom was out of the facility. The mom left through the spaceship playground. You can exit through that playground but can’t enter. (S2) was able to persuade her to leave that way.
LPA Whitmore obtained a copy of the letter that will be issued to the parent regarding Parent code of Conduct. Based on the investigation which included interviews and documentation. LPA Whitmore concluded the Case Management follow up visit. There are no Title 22 violations that have occurred, and no deficiencies. It was disclosed that ( S2) asked for the the children to be taken outside.( S3) Supervisor did contact the parent and there was no answer. The facility will be issuing a letter to the parent about Parent Code of Conduct. A meeting will be scheduled with the Parent.
A notice of Site Visit was given amd must be posted for 30 days. Exit interview conducted and report reviewed with Director Telma Rojas.
