On 02/27/2025 Licensing Program Analyst (LPA) Cristina Castellanos and LPA Judy Laureano arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegation. LPAs Castellanos and Laureano met with Licensee Angelika Getmanchouk and discussed the purpose of the visit. LPA Castellanos toured the facility and observed twenty (20) children in care with two (2) adult staff providing care and supervision during nap time.

During the investigation, LPAs Castellanos and Laureano received and reviewed the following document: children’s records.

Per LPAs record review it was found that six (6) out of ten (10) children’s files were incomplete; Physician’s Report LIC701 not available for review. Based on LPAs record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

There was one deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.

An exit interview was conducted, and Plan of Correction was reviewed and developed with the Licensee Angelika Getmanchouck. A copy of this report and appeal rights were discussed and left with the Licensee, whose signature on this form confirm receipt of these documents.
