On 5/5/2025 LPA concluded all investigative interviews with all relevant parties. LPA inspected the outdoor yard and it’s fence and did not see any areas in need of repair. Gate door was tested, and door opens and closes appropriately.
Based on interviews and multiple LPA’s observation of the fencing, the allegation of facility playground is not fenced properly is unsubstantiated. There is not enough information to prove or disprove the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the above allegations did or did not occur.
An exit interview was conducted with Teri Trujillo, Facility Director and a copy of the report was provided.
Upon receipt of this report, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. |
