On 03/6/2025, Licensing Program Analysts (LPA) Adrian Risher and Chartrice Johnson and Licensing Program Manager (LPM) Deborah Lowe met with Viviana Salas, Assistant Director. Madison Sanello, Director arrived while LPAs were conducting the inspection. The purpose of the visit was an unannounced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. Facility is currently licensed with a preschool license 197495270. There were 10 children present in the yellow room with 1 teacher and 3 children present in the green room with 1 teacher during this inspection. LPAs observed 5 additional children arriving in the green room during the inspection. This is an application for a Single License conversion to add a toddler component with a total capacity of 30; 12 Toddlers ages 18 months – 36 months and 18 Preschoolers ages 2 years – 6- years old. The child care center will operate Monday – Friday 7:30 a.m. – 5:30 p.m. LPA toured and measured the on-limits areas. The fire clearance was approved on 02/21/2025. The facility has a total of 3 classrooms. The Green Room will be dedicated to the toddler program. The yellow and orange room will be dedicated to the preschool program. The yellow room is located on the main floor downstairs. There are stairs located in the middle of the yellow room that leads to the 2 classrooms located upstairs. The green room is on the right and the orange room to the left upstairs. Preschool children will have access to 2 sinks and 2 toilets. Toddlers will have access to 2 sinks and 2 toilets.

Toddler Indoor measurements are:

The Indoor total measurements contain a total of 288.84 divided by 35 square feet, will not accommodate Applicant’s request for 12 children. Based on today’s measurement the facility capacity limit for indoor space is 8 toddlers.

Toddler Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total of 515.06, which will not accommodate Applicant’s request for 12 children. Based on today’s measurement the facility capacity limit for outdoor space is 6 toddlers.

Preschool Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total of 1037.52, which will not accommodate Applicant’s request for 18 Preschool children. Based on today’s measurement the facility capacity limit for outdoor space is 13 Preschool children.

The 2 preschool classrooms and outdoor play area was observed with no changes. The measurements were obtained during prelicensing visit on 06/14/2023.

The following was observed:

Parents will enter and exit from the main entrance in the front of the school.

Parents will sign children in and out manually on the sign in and out sheets located in the classrooms.

The facility has water dispensers where children can fill their personal water bottles for outdoor and indoor activities. There are operable carbon monoxide detector located throughout the facility. LPA observed fire extinguishers with an inspection date of 09/19/2022 . LPA observed First Aid Kits with the required materials including a handheld digital thermometer.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots for napping maintained in a safe condition. There is sufficient quantity and variety of age- appropriate play equipment in good condition. There are cubbies in all classrooms for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephones

service. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in an upper cabinet and locked cabinet under the sink in the kitchen. These items are inaccessible to children.

All toilets, handwashing and bathing facilities are maintained in safe and sanitary operating condition. Hot water temperature controls are maintained within regulation. Handwashing fixtures do not deliver hot water.

The licensee/applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the office located in the toddler outside play area . The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The restroom located upstairs is designated for children who are ill.

The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the emergency backpacks and inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the kitchen and inaccessible to children.

The program provides snacks. The children bring lunch from home to the school each day. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the refrigerator located in the kitchen. The refrigerator is equipped with a thermostat to regulate temperature control. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are posted in the yellow room for parents and visitors to review. Facility does have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. Assistant Director stated there aren’t any firearms or weapons on the premises.

Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861

LPA advised applicant of the following regarding the toddler Program:

Preschool Program with Toddler Component A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative.

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Areas around and under climbing equipment, swings, slides and other similar equipment has natural grass, wood chips and rubber platforms for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA did not observe any equipment or hazards in the outdoor activity area. There is adequate shade and benches for resting in the outdoor activity area. LPA did not observe any bodies of water. Assistant Director verified there are no bodies of water on the premises.

The following items were discussed:

The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Child Care Center testing deadline has not past.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. The applicant and director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any

The following corrections are needed before a licensure will be considered.
1. The changing table in the toddler classroom will be moved within arm each of the sink located in the classroom.
2. Obtain an waiver for preschool outside play area. Director will submit a new LIC200 to request 6 toddlers instead of 12. The waiver request for the toddler's outside play area will not be needed.
3. Confirm that all required people related to the license have a criminal record clearance on file and they are associated to the facility.
4. Verification that the fire extinguishers have been serviced

Corrections are due by 3/17/2025

An exit interview was conducted with plan of corrected items discussed. LPA provided Director with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
