25 | On 03/13/2025 at 12:55pm, Licensing Program Analyst (LPA) Devon Carus conducted a case management visit for increased monitoring. LPA met with Mayura Vhanakaware, Licensee and explained the purpose of the visit. LPA observed 5 children in care.
The purpose of the visit is to ensure the Licensee is maintaining a safe and comfortable environment for the children in care. LPA observed the facility to be clean, and orderly. Licensee will continue to provide care for the following ages: 12 weeks to 14 years of age.
Licensee confirmed her operating hours are 8am - 5:30pm, Monday through Friday.
LPA Carus reviewed the enrolled children's files, and observed the LIC 9224 forms regarding the recent Type-A violation, and Non-Compliance Conference signed by each parent. LPA Carus also inspected the recent back yard additions to the child care, and observed them to be safe and free of hazards.
LPA Carus also followed up with the licensee on her training courses discussed during the meeting on 2/14/2025. Licensee confirmed that she received the document from Licensing Program Manager Maureen Neal outlining which courses need to be completed to be in compliance. Licensee was advised that she can reach out to the department if she has further questions on these courses. LPA Carus also received the declaration regarding what the licensee learned from the Family Child Care Orientation.
Based on observations made by the LPA, no deficiencies will be cited today.
LPA did not observe any violations during today's visit. Facility will continue to be under increased monitoring on a quarterly basis.
