Licensing Program Analyst (LPA) Angela Panushkina conducted a subsequent complaint visit to follow up on an Unlicensed Care Complaint. LPA met with the Operator/Administrator and explained the reason for the visit.

Based on complaint review, the substantiated findings shall remain the same, however the civil penalty assessed on 07/23/2025 shall be dismissed.

The Operator/Applicant submitted an application on 03/21/2025 and worked with the department to be licensed. As of 10/22/2025, Abundant Senior Care - #197610790 was licensed.

Exit interview conducted and a copy of this report signed and delivered.
