25 | At approximately 1:00 p.m. on 12/30/25, Licensing Program Analyst (LPA) Nicholas Reed conducted an announced prelicensing inspection. LPA met with the administrator and applicants and disclosed the reason for the visit.
Today’s prelicensing inspection is an initial application without residents in care. It is a single story building with four (04) bedrooms, three (03) bathrooms, kitchen, laundry room, common areas, and outdoor areas. It has an approved fire clearance for one (01) ambulatory resident in Bedroom #1 and five (05) nonambulatory residents, of which one (01) may be bedridden in Bedroom #4. Approved hospice waivers for six (06).
LPA observed a maintained front yard with new rails and ramps leading to the main entrance. Postings at the front and in the hallway included COVID precautions, facility license, confidential complaints contacts, emergency disaster plan, rights of resident councils, theft and loss policy, personal rights, and the facility sketch.
Walls, floors, windows, screens, and blinds were clean and in good repair. At 1:10 p.m. LPA measured the room temperature to be 68 degrees Fahrenheit. The living room contained a television, reading material, exercise equipment, puzzles, art supplies, and furniture in good repair. A fireplace was appropriately covered and turned off. Nightlights were observed in common areas and walkways. The staff bathroom was clean and odorless. At 1:15 p.m. LPA called out from the house telephone which was deemed operational. WiFi, a tablet, and a computer were available for residents. At 1:20 p.m. the room temperature was measured to be 68 degrees Fahreheit.
