On 10/31/2025, Licensing Program Analyst (LPA) Justeene Tamayo conducted an unannounced Required 1-Year inspection, at the Almazan Family Child Care Home. Upon arrival, the LPA met with licensee, Laura Almazan who guided LPA on a tour of the facility. Family members residing in the home include 2 adults(licensee and licensee's spouse) and no minor children. All adults living in the house have been background cleared. Per the Licensee, hours of operation are Monday through Friday open for less than 23 hours. Per licensee, no overnight care is provided at the moment. Upon arrival, there was 1 preschool age child and 1 school age child in care with licensee. Incidental Medical Services (IMS) were discussed.

Physical Plant:
This is a single-story home with 3 bedrooms and 2 bathrooms. Per licensee, care is provided primarily in the living room, room #1 and backyard. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds(under kitchen sink with safety latch), medication(stored in master bedroom with key lock) and hazardous items(upper kitchen cabinet with safety latch) that can pose a danger to children. LPA observed age appropriate safe toys and napping equipment on the premises. Off limit areas include: the family room near kitchen(barricaded by mesh safety gate), kitchen(barricaded by safety gate) bedrooms #2-3(key locked), bathroom #2, laundry room and garage(sliding lock). The laundry room is located inside of the garage.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Main Care Area: Main care is provided in the in the living room (playroom), bedroom #1(sleeping room), and the backyard. Children use the bathroom located on the hallway on the left-hand side. LPA observed age-appropriate toys and furniture for the children. There are age-appropriate games and books on the premises. Per licensee, there is a designated area for ill/sick children in the living room. Children nap in the living room and bedroom #1. Cots observed in bedroom #1.

Fire/Health/Safety: There is a cell phone kept charged and on the Licensee at all times.
Smoke Detectors and Carbon Monoxide were observed to be in operable conditions.
The First Aid kit was observed to be complete with supplies including thermometer, tweezers, scissors, gauze, bandages, cleansing pad/solution, and a first aid manual. LPA observed a required fire extinguisher (3A40BC) reading in Green and currently serviced. The facility currently does not have childcare insurance. Per licensee, she currently does offer transportation, LPA observed insurance to be current.

Medications/ Hazardous Materials: Medications are stored in the master bedroom (off-limits). Cleaning compounds are stored in a lower cabinet in the kitchen, secured with a safety latch. Per licensee, there are no firearms in the home.

Incidental Medical Services (IMS): Incidental Medical Services (IMS) policy was discussed. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The Licensee will not be providing IMS to the children at this time.

Records: Licensee’s CPR/First Aid is maintained current and expires on 01/17/2027. Mandated Reporter Certificate is current and expires on 03/02/2026. Per Licensee, fire and disaster drills are conducted every 6 months; the last drill was documented and conducted on 10/17/2025. Per LPAs observations, the Facility License, Emergency Disaster plan, Earthquake Preparedness and Parents Rights Poster were posted.

Children's bathroom: Children use the bathroom located in the hallway on the left-hand side. The bathroom was clean, sanitized, and in good repair. The bathroom was toured and inspected sink and toilet which is in operable condition. LPA observed a key lock on the bottom of the bathroom sink.

Kitchen/Dining Room: The kitchen has a physical barrier(safety gate). Sharp items are located in an upper cabinet with a safety latch. Per licensee, she does currently have a food program. Meals offered: Breakfast, AM Snack, Lunch, PM Snack and Dinner.

Fireplace: The fireplace is located in the living room and was observed to be properly barricaded.

Backyard/Outdoor areas: The backyard is fenced in with cement block fence. The outdoor play area was inspected and was observed to be free of hazards, lose or sharp parts and tools. LPA observed appropriate and safe toys in the play area. Per licensee, there are two dogs in the property that are kept in the backyard (off-limits).

During the walk-through in the backyard, LPA observed the installation of chicken wire along the entire length of the wrought iron fence. The diameter gaps in the chicken wire are less than 1 3/4 in diameter. The chicken wire effectively reduces the vertical spacing between the bars to no more than 4 inches. The total height of the fence is now 5.2 feet, and the vertical clearance from the bottom of the fence to the cement is 2 inches. The swimming pool gate runs from one end to the cement wall, to the right side of the other cement wall. There are two gates each self-closing and self-latching, and opens away from the bodies of water. The backyard is completely fenced (brick wall), shaded area and a patio. The play area is clear and clean of debris, the play area is fenced and gated all around.


LPA also observed the new pool requirements that came into effect on 01/01/25. LPA observed a life ring with a diameter of 20 inches and is labeled and approved by the United States Coastal Guard located on the pool gate. LPA also observed a rescue pole with a body hook at 12 feet in height. Licensee purchased the ASTM International Standard F2208 pool alarm, and located inside of the bodies of water. LPA also observed licensee's daily pool log.

No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. MyChildCarePlan.org: Family Child Care Home [Applicant, or Licensee] was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Criminal Record Clearance: Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care.

Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within 24 hours of incident by telephone and in writing within 7 days of incident to the department. Licensee was informed all suspected Child Abuse should be reported to the Child Abuse Hot-line at 1-800-540-4000. The above incident should be reported on the form LIC624B per the regulation. The form should be email to unusualincidentreport@dss.ca.gov

There are no deficiencies cited during today’s inspection.

Exit interview conducted and report was reviewed with the licensee Almazan, along with her appeal rights and Notice of Site Visit.
