13 | On December 23, 2022, Licensing Program Analysts (LPA) Beneroso conducted a subsequent complaint investigation for the above allegations for the purpose of delivering the findings. Upon arrival LPA was greeted by Rushani Thomas-Wise who guided LPA on a tour of the facility. LPA observed 76 children in care and 22 staff members providing care and supervision in the facility. The investigation consisted of interviews with staff, and other complaint relevant parties including the review of supportive documentation.
Allegation #1: Based on the documentation reviewed and Interviews conducted with staff it was revealed the facility handled the plumbing repairs in a timely manner.
Allegation #2: Based on staff interviews and LPA's observations on the 10 day visit conducted on 10/03/2022, the facility did not have any unpleasant odors.
Based on the evidence obtained the preponderance of evidence has not been met; therefore, the allegations have been deemed to be unsubstantiated.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove the alleged violation occurred ; therefore, the above allegation is unsubstantiated. No deficiencies were cited.
An exit interview was conducted and a copy of the report was left with the Director Rushani Thomas- Wise.
