25 | On Tuesday, January 14, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management -Incident inspection, to follow up on a self reported incident that occurred on December 10, 2024 in regards a statement made by child #1 of being pinched by staff #1.
During this inspection, LPA Rivera interviewed staff #1, director, and child #1. During the interviews with staff #1, director and child #1, there's no indication of personal rights being violated.
Based on the information and documents provided, no deficiencies have been cited during this inspection.
A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report and appeal rights were reviewed with director Carmen Cerda.
