Licensing Program Analyst (LPA) Maddox met with Early Learning Supervisor, Celia Cassio, Site Director, Rosa Ayala-Lopez for the purpose of conducting a pre-licensing inspection. This center is located on the premises of Living Praise Church. Currently, this center is licensed for an Early Learning Head Start (EHC), staff are requesting to relocate the EHS component to the West side of the premises, and re-license the EHS classrooms to a Head Start program for children ages 2 - 5 yrs. During this Pre-Licensing inspection, LPA toured and measured 4 classrooms and the outside play area according to facility sketch. Center will operate Monday through Friday 7:00 am to 6:00.

INDOOR ACTIVITY SPACE:
· The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors
· Floors of all rooms have a surface that is safe and clean (wood flooring)
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (stored in locked cabinets under sinks in each classroom).

· There are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children:
· The isolation is located in the office for any child who becomes ill during the day.
· LPA observed operable carbon monoxide detectors and smoke detectors(hard wired). Fire extinguishers are located throughout the facility and fully charged.
· The center has a working telephone

The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

All 4 classrooms have equal measurements:
Classrooms #5 - 8: 31 X 32 = 992 sq ft /35 = 28 children
Staff are requesting to accommodate 16 children per classroom
Total indoor capacity: 112

LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. Toilets and sinks are functioning properly.

There are a total of 8 toilets and 8 sinks
Capacity of toilets and sinks can accommodate 3 120 children.
There is a Staff bathroom located in a building adjacent to the classrooms, this bathroom will also be used as an isolation bathroom.

The play yard was free of hazards including, but not limited to, holes, broken glass and other debris. As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls (rubber matting). Drinking Water will be brought out in the form of water pitchers and/ or disposable cups and water bottles.

There are 2 outside play yards.

Measurements were as follows:


Play yard #1: (front of classrooms)
Area in front of classroom #7: 47 X 36 = 1,692/75 = 23 children
Area in front of Classroom #8: 1,558/75 = 21 children
Play yard #2: Located in the rear of classroom # 6: 450/75 = 6 children
Total Outdoor space: 50

There are 2 swimming pool located in front of classrooms #7 and #8 with regulated 5 ft. fencing which includes a spring closure on the gate.

There are refrigerators located in each classroom however CCRC has their own food program which will deliver breakfast, lunch and snacks daily. Some snacks will be kept on premises.



NAPPING

· Cots used for napping were maintained in a safe condition.


· Each cot/mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· LPA informed applicant Cots or mats should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Bedding/Linen will be stored in cubbies.

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children.

A refrigerator shall be used to store any medication that requires refrigeration.

Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

**Parent Board is located at the main entrances of each room. Fire extinguisher, first aid kit, carbon monoxide detector and fire alarm are all located at each entrance. LPA observed required forms posted. Staff are certified in Pediatric CPR and First Aid. Applicant was informed to conduct and maintain Fire Drill/Earthquake logs. Applicant is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B. There is a binder located at the entrances for signing children in and out, center also utilizes an electronic sign in an out procedure called Child Plus.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Applicant states the building has been tested for Lead.


There is an approved fire clearance for the requested capacity of 64 Preschool children. As a result of the unannounced Pre-licensing inspection, center is found to be in compliance with Title 22 Regulations and ready for licensure. Exit interview was conducted and a copy of this report was given to Early Learning Supervisor, Celia Cassio and Center Director, Rosa Ayala-Lopez.
