Licensing Program Analyst (LPA) Maddox met with Pastor Darrell Maston and Melanie Rodriguez, Director today for the purpose of conducting an announced Pre-Licensing inspection for a PS component. This facility is located within a strip mall inside the Grand Central Collective Church. Applicant is requesting to care for 90 children ages 2 - 5 yrs. The hours of operation will be: Mon through Friday from 7:00 am to 6:00 pm.

INDOOR ACTIVITY SPACE:

· The child care center was toured and found to be clean, safe, sanitary to ensure the safety and well-being of children, employees and visitors


· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available water fountains (LPA reminded applicant to clean the water fountains several times a day to slow the spread of virus') Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (stored in high cabinets. Stored in a locked closet
· There are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children.

The isolation is located in a room within the "Green Room" for any child who becomes ill during the day.
· LPA observed hard wired smoke detectors and fully charged Fire extinguishers located throughout the facility.
· The center has a working telephone
· Sign-in and out procedure will take place through the Brightwheel app.
· The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.
OUTDOOR

LPA observed a shaded rest area for the children )Pop-up tents an natural shade from large trees. The entire yard is surrounded with fencing. There are no bodies of water on the premises

The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls (padded artificial turf).

RESTROOMS

There are 2 bathrooms designated for children and will be exclusively for children during the hours of operation. All toilets and hand washing facilities shall be maintained in safe and sanitary operating condition.

Staff bathrooms are located on the second level.

SUPERVISION: Applicant shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation.

FOOD SERVICES

Facility will no serve lunches, snacks only. The snack menu must be posted for parents’ review.

· Pesticides and other similar toxic substances were not stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies.


· Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.
· Trashcans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
· There is a refrigerator available to store any medication that requires refrigeration.
· All foods shall be selected, transported, stored, prepared and served so as to be free from contamination and spoilage and shall be fit for human consumption. Food in damaged containers shall not be accepted, used or retained.

POSTINGS:

Applicant was informed of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

CHILDREN’S RECORDS:
LPA explained all children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours.
TRANSPORTATION:

Facility will not provide transportation.

NAPPING

· Cots used for napping were maintained in a safe condition.


· Each cot shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· LPA informed Applicant Cots should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Bedding/Linen will be stored in cubbies.

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration.

Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Outside play space:
97 X 26 = 2,522/75 = 34 children
There is an outside play schedule that shall remain posted during the hours of operation

Fire Clearance has been received for the requested capacity of 90 children, however with LPA's measurements taken today, center can accommodate 77 children. Waiver is needed for the outside play yard.

Needed Prior to Licensure: LPA did not see any carbon monoxide detectors at the time of this inspection, Applicant is advised to purchase carbon monoxide detectors and place them in areas where children will sleep higher on walls. LPA noticed the carpet is unraveling in the Purple and Red Room, Applicant should place some type of runner or area rug over these areas until carpet can be replaced. There is a stage area in the purple room that has some areas where the plywood is coming apart, this should be repaired as well, LPA is also recommending this room be occupied by older PS children. Toilet inserts are needed for the adult size toilets, step stools are needed for the sinks in bathrooms and water fountains. Applicant was informed that the water must be tested for lead 180 days after licensure. Exit interview conducted, copy of this report left at facility with Applicant.
