Licensing Program Analysts (LPA) Maddox and Pittman conducted an announced visit today and met with Vonda Perry and Rivka Jones for the purpose of conducting a Pre-Licensing inspection for a Preschool component for children ages 2 through 5 yrs . Applicant is requesting to be licensed for 30 PS children. The hours of operation are Monday through Saturday from 5:00 am to 8:00 pm. This center is located within a strip mall. Applicant has completed the application with local Building and Safety to convert classification from "B" to "E". There is 1 large room designated for Preschool children.

INDOOR ACTIVITY SPACE:

· A comfortable temperature for children shall always be maintained.


· Furniture and equipment are maintained in good condition, free of sharp, loose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available in the form of a water cooler and disposable cups
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children. Stored in the suite next door in a locked cabinet, under the sink.
· There are fully stocked first-aid kits in locations accessible to staff but inaccessible to children: the kit is missing a thermometer.

  • The isolation area will be the front office for any child who becomes ill during the day
  • LPAs observed working carbon monoxide detectors and smoke detectors

  • · The center has a working telephone
    · Sign-in and out procedure will be conducted manually.
    · The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.
OUTDOOR

The play yard is surrounded by wrought iron fencing. Staff will bring drinking water out for children. There are no bodies of water on the premises. LPAs observed very minimum outside play equipment and shade.

RESTROOMS

There is one restroom available, the restroom has 2 toilet and 2 sink. All toilets, hand washing, and bathing facilities shall be maintained in safe and sanitary operating condition. There is also a separate staff bathroom in the suite next door, also licensed by the same Licensee.

SUPERVISION:

No child(ren) shall be left without the supervision of a teacher at any time, Supervision shall include visual observation.

CHILDREN’S RECORDS:


All children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours.
HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children. A refrigerator shall be used to store any medication that requires refrigeration. The refrigerator is in the suite next door.

Licensee has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

NAPPING

· Cots used for napping were maintained in a safe condition.


· Each cot shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· LPA informed applicant Cots or mats should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Bedding/Linen will be stored in 8 individual cubbies.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. POSTINGS:

The licensee shall post signs at the entrance to the child care center that provide the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available

Needed Prior to Licensure:

NOIA letter was reviewed and emailed prior to the inspection conducted today.

Add additional cots to capacity

Add additional cubbies for children belongings

Acquiring a thermometer for the first aid kit

Missing/incomplete paperwork: LIC 308 for Akemie Jones; Updated LIC 500 for both facilities. LIC 501 for Akemie Jones; LIC 503 for Akemie Jones; Operations and Record Keeping for Akemie Jones; Immunizations: MMR, Tdap, TB, and flu vaccination/statement for Akemie Jones ; LIC 610 with 2nd relocation site;Job Descriptions incomplete-missing ECE requirements, qualifications and lines of supervision; Parent Handbook- admission policies include forms needed for children's records & missing days and hours of operation; Admission Agreement- see section 101219 and include all of section (b) and signature lines; Employee checklist missing Tdap immunization & flu statement/vaccination; List of furniture/play equipment; control of property.

Applicant must ensure to put the license number on all advertisements.

Fire Clearance has been received for a capacity of 30 PS children, however, during this inspection the measurements will accommodate a capacity of 23 PS children.

Indoor measurements: 815 divided by 35 sq ft is 23.

Outdoor measurements: 822 divided by 75 sq ft is 10. The outdoor play area requires a waiver because the outdoor measurements do not meet the requested capacity.

An exit interview was conducted and a copy of the report was read and provided to the Applicant.
