On 04/14/2022 this facility was cited for: California Health and Safety Code: 1569.38 (b)(1) Posting of licensing reports; disclosure to new residents. (b) A licensed residential care facility for the elderly shall provide written notice to a resident, the resident’s responsible party, if any, and the local long-term care ombudsman, within 10 days from the occurrence of either of the following events: (1)The department commences proceedings to suspend or revoke the license of the facility pursuant to Section 1569.50. Licensee failed to disclose a written notice to a resident, the resident’s responsible party and the local long-term ombudsman within 10 days from the occurrence.

On 04/28/2022, LPA Irra conducted a Plan Of Correction (POC) visit to follow up on the citation noted during the 04/14/2022 visit. As of the 04/28/2022 visit, DEFICIENCY WAS NOT CLEARED. Civil Penalties were issued for period 04/19/2022-04/28/2022. Total of 10 days, at $100.00 per day. The amount noted on the issued Civil Penalty (LIC 421FC) was $ 1,000.00. A civil penalty of $100 per violation per day to be assessed until corrected.

As of today, Facility Administrator provided evidence to LPA Vasallo (visit completed on 05/03/2022) that the notification was sent to families about the accusation. Evidence provided included the letter sent to families along with copies of certified mail certificates and receipts.. During today's visit, LPA obtained contact information for residents authorized representatives. Once verification with resident/resident’s authorized representative is completed, it will be determined if another citation will be issued.
