Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Random Annual inspection to the facility. LPA met with the Licensee Karene Gainer and informed her of the purpose for the visit. LPA provided the inspection Entrance Checklist (LIC 126). Licensee guided LPA on a tour of the facility. There were 4 children present including 2 infants. Also present was Karevina Gainer, Assistant. All adults present during the inspection have obtained a criminal record clearance or exemption. Individuals residing in the home have been discussed and noted. Licensee provides care to children ages newborn to 12 years and conformed the hours of operation are Monday through Friday 7:00AM to 5:30PM.

This is a single-story home which consists of 3 bedrooms and 2 bathrooms, kitchen, living room, den (converted into napping room), and garage (converted into activity/daycare room).

Areas used by the children include the kitchen for eating, 1 bathroom (located near daycare room), napping Room (located next to kitchen), and daycare room (located to the rear of the home).

Per Licensee, areas off limits to children and parents include living room, all bedrooms, and 1 bathroom. LPA observed entrance to facility is through the side yard. LPA observed a locked door in kitchen blocking access to rest of the home making areas inaccessible to children in care.

LPA reminded Licensee that children do not eat or sleep in the converted garage used as a daycare/activity room. LPA observed the daycare/activity room to have central air and heat and does not have a door that would allow a car to be parked in the garage.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months)
Staff records were reviewed and found completed.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Based on the LPA's observations and records review 1 "B" deficiencies will be cited today in accordance with California Title 22 Regulations.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee, confirmed that there are no Registered Sex Offenders (RSO) living in the facility and LPA completed the RSO search in Megan’s Law Website.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Karene Gainer. Appeal Rights discussed and noted. ----End of Report

LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted in on wall by door where parents drop off and pick up in daycare activity room. LPA reviewed completed facility records including LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and was serviced on 7/30/24 Licensee was reminded that fire extinguisher needs to be serviced yearly. The home maintains telephone service via cell phone line. The home is observed to be clean and orderly. There are toys and other age-appropriate material available. LPA observed that cleaning compounds are in kitchen inaccessible to children in care. The bathroom that children use is in the hallway by the daycare/activity room and observed to be clean and free of hazards.

Licensee states that there are insecticide and poisons locked in off limits area not only inaccessible to children. Per Licensee there are no firearms or weapons stored in the home.


The facility does not have a pool or similar bodies of water. There are no pets in the home.

Isolation area for sick children waiting to be picked up is in the Daycare room, supervised and away from the other children.

There facility has infants in care. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Currently, children are using the side and back yard for outdoor play. The side yard and back yard are separated by a gate and observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with including a play structures anchored to the ground, slides and climbing equipment is observed to have cushioning materials under structure. LPA did not observe any objects that could be hazardous to children in care.
