Licensing Program Analysts (LPAs) Nolan Tcheng and Joanne Solorio Campos conducted an unannounced Case Management inspection to follow up on incident reports submitted to the Department. Upon arrival at 10:45am, LPAs met with Director Brenda Trejo, to whom the purpose of the inspection was explained. A tour of the facility was provided. There were children present during the time of inspection.

Census was taken. There were 7 staff with 10 children.

On 10/03/2024, 10/11/2024, and 10/21/2024, there were three incident reports submitted to the Department regarding child personal rights being violated by person in care. During the inspection today, LPAs interviewed four staff members and obtained documentation in the form of ouch reports, pictures, and incident reports. Licensing staff reviewed the information provided from staff and from the documents. There was no previous history of child behavior that would have indicated need for increased precautionary measures. Following the first incident, on 09/30/2024, facility addressed the incident and responded with changes to classroom.

Incidents that took place following 09/30/2024,were observed by facility staff and were also incidents where the staff took action to intervene as quickly as they could. Staff meetings have taken place that have addressed the incidents and how staff will be engaging, moving forward.

After review of the information above, it was determined that facility and staff have imposed preventative measures to intervene before incidents occur. Supervision was maintained during the incidents. LPA discussed the importance of maintaining supervision to help prevent future incidents. Given the age of children in care, the behavior is appropriate for the age. LPAs determined there was no violation of personal rights.

No citations are being issued during today's inspection.

A notice of site visit was given and must remain posted for 30 days.
