| Information from the complainant indicates that facility did not prevent Child #1 from getting hurt from Child #2 while in care.
Interview with the director indicates that there has been an incident recently regarding Child #1. Incident took place on 06/08/2023. Based on documentation reviewed, the incident that took place on 06/08/2023, does not corroborate what was in the allegation reported. LPA reviewed additional incident reports from close in recency to complaint being reported. A parent conference regarding Child #1 and Child #2 was conducted to help facilitate steps moving forward for the safety and care of facility children based on statements made from parent. Reports were created by the facility for incidents that have taken place.
Staff interviews did not disclose any consistent cases of a child being injured by another child. Staff #2 did corroborate the incident of Child #1 and Child #2 that the director stated. Upon review of incident reports regarding the two children involved, it did not support the allegation being reported. LPA conducted further review of any incident/injury reports that may have involved Child #1 and/or Child #2

Based on information stated above, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
