On 5/30/2023 Licensing Program Analyst (LPA), Carolyn Tuba conducted an unannounced case management to amend complaint reports from 5/12/2023 and issue a citation. LPA created a LIC 9099 in order to generate an LIC 9099-D page to issue citation. Covid-19 risk assessment was conducted. LPA was greeted by Director, Christine Garcia and guided LPA on a tour to gain the census of 42 children in care with 6 staff. LPA conducted consultation with Director.

The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed.

Exit Interview was conducted and appeal rights (LIC 9058 03/22) were given, along with a copy of this report was provided to the Director, Christine Garcia.
