25 | Licensing Program Analysts (LPA), Anomeh Eivazian and Suzana Safaryan, conducted an unannounced case- management inspection to the above facility on 02/12/24 at 12:00 PM. LPAs met with Rohini Nimali David facility director who guided LPAs on a tour of the facility. During this inspection there were total of 53 children were present in the facility with nine staff.
Based on four staff interviews on this date, on 02/05/24 due to water leak and flood in classroom#2, all parents were contacted and school was closed on 02/05/24.
Based on four staff interviews on this date, due to water leak in classroom #2 on 02/05/24, classroom#2 children occupied classroom#5 (kindergarten classroom) on 02/06/24 and 02/07/24, which is not a licensed classroom by licensing.
Facility did not report 02/05/24 facility closure and temporary classroom#2 use of classroom#5 on 02/06/24 and 02/07/24 to the department.
The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with Joyce Young, teacher at 4:00 PM.
