25 | Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Annual Random Inspection on this date. Upon arrival LPA met with Jessica Cardenas, Licensee, informed of the purpose of inspection and provided form LIC 126, Inspection Entrance Checklist. The Licensee provided tour of facility. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access. Per licensee, the current hours when care provided are Monday – Friday 8:00am-5:00pm. here were 7 children present, including 4 infants. Also present was Scarlett Llamas, Assistant. All adults present have obtained the required background clearance and are associated to the facility. Individuals residing in the home were discussed and noted.
This is a single story home which consists of 3 bedrooms, 2 bathrooms, there is a detached garage and a storage shed observed to used to storage, both locked. LPA observed a parked RV in a a gated off area behind the garage.
Areas accessible to children in care are: Living room used as activity Room, dining room used as infant room, bathroom, and backyard play area.
Per Licensee, areas off limits to children and parents include: All bedrooms, laundry area, detached garage gated off area behind the garage and storage shed. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:
LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted in wall by entrance to home. LPA reviewed Children’s Roster form LIC 9040 and Emergency Disaster Plan (LIC 610A) ------Page 1 – Report Continues
