Per LPA observations, one of the toilets in the bathroom with the sign on the door that states boys is not flushing. Per interviews it was stated that 2 of the toilets were clogged recently and not working and some times they don't have enough or are out of supplies like paper towels and soap.
Teacher- Child Ratio: Facility is out of ratio- Reporting party stated that the facility does not have enough staff and is out of ratio and on 09/23/2024 there were over 20 children alone with 2 staff. Per LPA review of documents and interviews conducted, on 09/23/2024 the facility was over ratio by having about 30 plus children alone with two teachers. Per interviews it was also stated that the facility is constantly out of ratio and staff are confused by the ratio the facility follows due to the facility following Title 22 and Title 5 regulations.
Based on LPA observation, documents reviewed and received and Interviews conducted, the allegation of facility is out of ratio and physical plant is determined to be substantiated at this time. California Code of Regulations (Title 22, Division & Chapter) relating to this allegation has been cited on LIC 9099-D.
LPA Reyes informed licensee Director Aysegul Inceogl, that this report dated 10/01/2024 document(s) two (2) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Reyes informed the licensee Director Aysegul Inceogl to provide a copy of this licensing report dated 10/01/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
