On December 01, 2023, Licensing Program Analysts (LPAs) Kruz Long and Monique Ayala conducted a subsequent unannounced case management inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPAs met with lead teacher, Esmeralda Franco who guided LPAs on a tour of the facility. LPAs observed 11 children with 2 staff members.

The purpose of the visit is to follow up on an incident that occurred on 10/29/2023 and was reported to the department on 10/30/2023 (reported timely). The self reported incident is regarding personal rights.

During the initial site visit on 11/09/23, LPAs interviewed Staff #1 (S1), Child #2 (C2) to Child #4 (C4) and obtained the contact information for Child #1 (C1).

During today's visit, LPAs interviewed Staff #2 (S2).

Based on interviews with S1 and S2 and C2 to C4, there were no corroborating information to determine that a personal rights violation occurred. C1 was not interviewed due to parent request. The facility is not being cited any deficiencies today.

An exit interview was conducted and a copy of this report was provided to lead teacher, Esmeralda Franco. A Notice of Site Visit was provided; Notice of Site Visit must be posted for 30 days.
