On August 13, 2025, Licensing Program Analyst (LPA) Monica Ruiz conducted an unannounced required 3-year inspection at the facility noted above. LPA was greeted by Licensee Maria Flores, to whom the reason for visit was explained. LPA observed 2 preschool age children in care. Licensee states there are 9 children enrolled at this time, no infants. LPA reviewed 2 children’s files and 3 staff files. Adults in the home were discussed and all have fingerprint clearances.

Hours of operation are Monday through Friday, 5:30 AM to 11:00 PM.This is a single story home with 4 bedrooms and 2 bathrooms, living room, dining room, kitchen, fenced rear yard and front yard and an attached garage. Licensee was made aware that the garage cannot be used for napping or eating and should only be used for limited child care activities. Licensee Maria Flores states that child care takes place only in the enclosed front yard and patio area, living room for napping, and attached garage and restroom located inside attached garage. Sick/isolation area is the garage/activity room.

Licensee states that the backyard is being repaired at this time and is off limits to children. Access to the backyard from the front patio/child care area is made inaccessible by large metal gates on either side of the house. LPA reminded Licensee M. Flores that all construction or changes to the facilities physical plant are to be reported to licensing before repairs and/or construction begins on the facility.

The home is clean, orderly, comfortable, and well ventilated. LPA observed two children in care playing with a water activity table. Licensee and assistant were reminded to immediately dump out water as soon as water activities are over, to ensure the safety of the children in care. page 1/4

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are several age-appropriate and washable toys available. There’s a play structure with a slide in the front yard. There is also a play house and play kitchen. There are several tricycles and riding cars available for children. LPA observed blocks, puzzles, and sensory toys available for children in care. Napping equipment in the form of cots were inspected and were in good condition at the time of this visit.
Licensee's poisons, detergents, cleaning compounds, medications and other items which could pose a danger to children are stored in the laundry room behind a closed door with a child proof door knob. LPA advised Licensee Flores to move cleaning products to a high shelf, where they are inaccessible to children.
LPA observed a working smoke detector, Carbon Monoxide detector, and fully charged fire extinguisher in the day care activity room/garage. Fire extinguisher was last serviced on 9/26/2024 and there is a land line that is maintained at the facility. Licensee was reminded to have the fire extinguisher serviced or replaced annually.

There is a first aid kit on the premises. Per the licensee, there are no firearms on the premises. The Licensees and the assistants have current CPR and first aid that expires on 6/7/2027.

Licensees and all assistants have current mandated reporter certificates on file. Licensees were reminded to maintain copies of their immunization records for themselves and all employees and volunteers, on file at the facility.


All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years, www.mandatedreporterca.com,

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCL office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

The licensee was informed that the presence of adults in the home without Criminal Record Clearance or Exemption will be cited and civil penalty assessed for $100 per day. The licensee may find additional information and forms on the DSS website at www.ccld.ca.gov including information on the Live Scan application (LIC 9163). Appointments can be made for Live Scan at 1-800-315-4507.

Per the licensee, fire and disaster drills are conducted every month; last drill documented and conducted on 04/19/2025.

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Emergency Disaster Plan (LIC610a), Notification of Parents' Rights Poster (PUB 394).

The following was discussed with the licensee:


Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. Licensee was reminded that supervision is always required to children in care.

Licensee was informed that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.
