Licensing Program Analyst (LPA), T. Tran arrived at the above licensed facility for the purpose of concluding the investigation of the above complaint allegation. Upon arrival, LPA met with Superintendent, Jennifer Hartl.
Based upon the evidence obtained during the course of the investigation through interviews and record reviews. 12/25/25, around 1:22PM, during naptime staff confirmed that C1 (see LIC 811) likes to cover the head when napping and staff failed to remove the blanket by allowing the child napping with a blanket covered the head until the parent arrived early and observed such incident. Based on the facts presented, this incident was the result of Title 22 Personal Rights Violation, therefore, the above personal rights allegation that staff did not provide a safe environment for children while napping was substantiated. A finding means that the complaint is substantiated, and the allegation is valid because the preponderance of the evidence standard has been met.

Licensing Program Analyst (LPA), T. Tran arrived at the above licensed facility for the purpose of concluding the investigation of the above complaint allegation. Upon arrival, LPA met with Superintendent, Jennifer Hartl.
Based upon the evidence obtained during the course of the investigation through interviews and record reviews. Staff indicated that C1 did not have any serious injuries while in care and children do have access to water when needed. C1 is fully potty trained and able to use the bathroom independently. In addition, staff do communicate with parents regarding the children's health and safety. Therefore, all the above allegations have been determined unsubstantiated.
A notice of site visit was given and must remain posted for 30 days.
