On 02/03/2025 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced annual inspection at the above facility. Licensing staff met with Zenobie Marshall, whom the reason of the inspection was explained. Licensee guided licensing staff on a tour of the facility. One child was present during the visit. Per Licensee, there are 8 children currently enrolled. Facility hours are Monday-Saturday 4:00am-11:50pm.

This is a 2 story home which consists of 3 bedrooms, 1 1/2 bathrooms, kitchen, laundry room, living room, dining room, front yard, and backyard (fenced). Currently residing in the home are two adults (criminal record clearance on file) and one minor children.

Areas accessible to children were inspected as follows: Kitchen, dinning room, living room, ½ bathroom downstairs and backyard.
Areas off limits include: Second level is inaccessible to children; LPA observed the stairs leading to the second level has a fence making it inaccessible to children. Per licensee overnight care is provided. LPA observed cots for children to use for napping and overnight care.

LPA observed the following required posted documentation in the main entry way of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA reviewed facility records for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan. LPA observed disaster drill conducted on 01/13/2025.

There is central air and heating. There is a land line in the home. Smoke and carbon monoxide detectors were tested and are operable. Licensing staff observed the valve on the required 2A 10BC fire extinguisher indicates fully charged with a receipt dated 01/2025 as proof of purchase.

Currently children use backyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. Page 1 of 3

LPA did not observe any objects that could be hazardous to children in care. Isolation area for sick children waiting to be picked up will be in the living room, away from the other children. Per Licensee there are no firearms or weapons stored in the home. Licensee provides breakfast, lunch and dinner. Licensee transports children before and after school.

Child’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights.



Licensee file was reviewed for the following: LIC 508- Criminal Record Statement, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Pediatric First Aid and CPR certification on expires 09/01/2026 for licensee. Mandated Reporter Training has been completed and expires on 05/11/2025 for licensee. www.mandatedreporterca.com

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. Licensee does not have any infants enrolled. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a plan for providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on Licensing staff observations and records review, no deficiencies are being cited, licensee is in compliance with California Code of Regulations Title 22.



A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
