An unannounced Complaint Investigation was conducted by Licensing Program Analyst (LPA) Stephanie Li to conduct deliver findings. Risk assessment was made. Upon arrival, LPA met with Director Gracie Chavez, who guided LPA on a tour of the facility. Census was taken. There are 101 children and 12 teachers present during visit.

During the investigation, LPA interviewed director Gracie Chavez and 6 teachers. Licensing staff also obtained several documents related to the complaint allegation, including but not limited to, a copy of the Facility Roster, obtained updated copies of schools sick policy and parent handbook to include Title 22 regulation requirements and designating the office as the isolation area. Licensing staff also recieved copies of all Classroom app messages to parents notifying them of sick children and reminders of sick protocols. Licensing staff also observed temporary isolation areas in each of the classrooms (house 1, house 2, house 3, and house 4) while ill children are waiting to be retrieved by admin staff.

Based on observations made and statements obtained from interviews, no disclosures were made to corroborate the allegation or evidence to support the allegation. Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with Director Gracie Chavez.
