Licensing Program Analysts (LPAs) Veronica Martinez Garza and Priscilla Ochoa conducted an unannounced Annual/Random inspection at the above facility on 04/19/24 at 09:10 a.m. LPAs met with Susana Alonzo, licensee who guided analysts on a tour of the facility. There were 04 children during this inspection. Present during this inspection was the Licensee’s husband/assistant Alejandro Villagran. Per licensee, 10 children are enrolled. Operation hours are Monday – Friday 06:00AM to 06:00PM.
This is a one-story home which consists of 3 bedrooms, 2 bathrooms, living room, dining room, kitchen, back patio, garage (converted playroom), and backyard. Areas accessible to children include living room, dining room, kitchen, bathroom (located between kitchen and back bedroom), back patio, garage, and backyard. Areas off-limits to children and parents include 3 bedrooms and 1 bathroom. Per licensee, the garage is only used for activities. Per Licensee, family members residing in the home are 2 adults and 1 minor. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home. Individuals living in the home are identified on the attached LIC811.
LPAs toured all areas identified on the facility sketch used by children during this visit and were inspected for safety, comfort, and cleanliness. LPAs observed safe toys, play equipment and materials for children. LPAs toured the living room, dining room, kitchen, bathroom, garage, back patio, and backyard. LPAs observed appropriate napping equipment. LPAs observed a play pen, cots, and mats. LPAs did not observe any hazardous materials throughout this inspection. There’s also a first aid kit readily available. LPAs observed the bathroom children use to be safe and in sanitary conditions. LPAs toured the kitchen and observed that the kitchen has child proof locks.
LPAs observed the following required posted documentation in the dining room of the facility: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form.
