25 | On March 22, 2024, at 9:30am, Licensing Program Analyst (LPA) Dayna Chambers, conducted an unannounced case management visit for the purpose of ensuring the facility is maintaining compliance in the areas previously cited on 05/30/23. LPA requested a copy of the lead testing report, Director’s package, and reviewed unusual incidents for dates 01/29/23, 09/15/2023, 10/13/2023, 02/21/24. Upon Arrival, LPA met with Dena Hardin, Director. LPA observed 40 children with 6 staff in care upon arrival. LPA toured the facility with the Director at 10:30am. LPA observed three classrooms: Bungalow (ages 2-3 years) with 9 children and two staff, Garden Room (ages 3-4 years) with 14 children and 2 staff, and the Sunroom (ages 4-5 years) with 17 children and two staff plus Director is present. There was a total of 40 children and 6 staff present. The centers hours of operation are 8:00am to 5:00pm Monday through Friday. This center served AM and PM snacks and parents provide their child’s meals. LPA obtained the Director Package, children’s roster, reviewed staff, and children’s files, and discussed unusual incidents outcomes. All unusual incidents were reviewed and found to be resolved. All children are still enrolled at the center. While at the center, LPA reviewed two pages of a lead report and it was found to be incomplete. LPA advised the center to conduct the lead testing and ensure all water sources are tested and labeled for review. Director will provide the report to LPA after contacting the licensee.
Exit interview was conducted with Director Dena Hardin. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
