An unannounced case management inspection was conducted today by Licensing Program Analysts (LPA) Crystal Green and Lissete Gonzalez to inspect and measure facility for capacity determination. Licensee is requesting to decrease preschool capacity and add a toddler option program. Due to COVID- 19 precautionary measures were taken, individuals present during inspection wore appropriate personal protective equipment. Facility was observed to be adhering to local public health guidelines. Upon arrival, Licensing staff met with Family Service Specialist, Jacqueline Godina and Lead Techer, Raquel Barbara, who assisted until Assistant Program Director, Carol Gregory arrived to resume providing a tour of the facility.

During today’s visit, it was disclosed that the applicant will be updating the LIC 200A application form with a change to request 0 Preschoolers and 18 Toddlers; Original application request indicated a request for 20 Preschoolers and 16 Toddlers. Based on the new request to serve Toddlers (18-36 months), prior to providing care to preschoolers, Licensee acknowledges that Community Care Licensing will be notified. Licensee is choosing to provide care for only the toddler age group. Currently, the facility is licensed for 40 Preschoolers.

There were no children present during today’s visit. The proposed Toddler classrooms are Room #1 and Room #2. Licensing staff observed age appropriate equipment, toys, and supplies for children. The Toddlers designated isolation is located in the office area. A cot is available for ill children waiting to be picked up.

Per Facility Representative, linens and bedding are laundered weekly and as needed by a staff member. There are changing tables located in Room #1 and Room #2 within arm's reach of a sink. LPA observed a landline in each classroom. There are emergency supplies and a first aid kit located in Room #1 and Room #2. There is drinking water available in form of a water dispenser with disposable cups. There are trash cans with tight fitting lids available in the classrooms. There is a restroom located in each classroom. There is a parent board with the required postings at the entrance to the facility. Report Continues Page 1 of 3

LPA observed snack and meal menus in the classrooms. Facility provides Breakfast, Lunch, and PM snacks. Food is brought in daily by a caterer and meals are maintained warm through the food warmer.

The Toddler outdoor playground was inspected and measured. There is age appropriate play equipment in the playground. The outdoor space is fenced. Drinking water will be available to children via a water pitcher and disposable cups. There is adequate shade in the yard. Licensing staff observed the structure poles located around the shade area to be missing protective cushion barriers to prevent possible injury.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on the LPA’s observation, the following corrections need to be corrected prior to obtaining the capacity decrease with toddler option child care license. Corrections are due by 11/30/2021:


1. Licensee is requesting to serve 0 Preschoolers and 18 Toddlers. Licensee will submit a new LIC 200A Application reflecting the new request.
2. Installation of protective cushion barriers to the four (4) shade structure poles located on the outdoor playground.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
